老年性骨质疏松血瘀病机的宏观论证

【摘要】 目的:通过对老年性骨质疏松患者“血瘀”表现的宏观指标观察和分析,论证老年性骨质疏松症存在血瘀的改变,为运用活血通络法治疗老年性骨质疏松提供理论基础。方法:对30例老年性骨质疏松症患者、30例老年性低骨量进行“血瘀”临床表现综合评分和甲皱微循环评分,并进行血液流变学检测,同时检测30例健康成年人作为正常值对照,并将血瘀综合评分与甲皱微循环评分、血液流变学检测和骨密度进行相关系数分析。结果:1.老年性骨质疏松症患者组、老年性低骨量组、健康成年人组间的骨密度有非常显著性差异(P<0.01),表明老年性骨质疏松的骨密度明显低于青壮年,老年同龄之间亦有非常显著性差异。2.老年性骨质疏松症患者组“血瘀”临床表现评分与健康成年人组比较,有非常显著性差异(P<0.01);老年性低骨量组“血瘀”临床表现评分与健康成年人组比较,有非常显著性差异(P<0.01);老年性骨质疏松症患者组“血瘀”临床表现评分与老年性低骨量组比较,有显著性差异(P<0.05),表明老年性骨质疏松患者有痛有定处、舌下脉络曲张、舌质紫暗有瘀斑、口唇暗红皮肤粘膜瘀斑等血瘀证的表现,其“血瘀”临床表现评分与老年性骨质疏松程度呈正相关性。3.老年性骨质疏松症患者组甲皱微循环综合评分与健康成年人组比较,有非常显著性差异(P<0.01);老年性低骨量组甲皱微循环综合评分与健康成年人组比较,有显著性差异(P<0.05);老年性骨质疏松症患者组甲皱微循环综合评分与老年性低骨量组比较,有非常显著性差异(P<0.01),表明老年人骨质疏松患者的甲皱微循环综合评分与老年性骨质疏松程度呈正相关性。4.老年性骨质疏松症患者组血液流变学各项指标与健康成年人组比较,有非常显著性差异(P<0.01);老年性低骨量组血液流变学各项指标与健康成年人组比较,有显著性差异(P<0.05);老年性骨质疏松症患者组血液流变学各项指标与老年性低骨量组比较,有非常显著性差异(P<0.01)或显著性差异(P<0.05),表明老年性骨质疏松患者存在血液流变学变化,其血液流变学各项指标与老年性骨质疏松程度呈正相关性。结论:1.老年性骨质疏松症属中医“骨痿”、“骨痹”范畴,是伴随人体的衰老而发生的疾病,老年性骨质疏松症患者由于其生理病理过程的原因,血瘀在其发病有着重要作用。2.老年性骨质疏松症患者均存在着痛有定处、舌下脉络曲张、舌质紫暗有瘀斑、口唇暗红皮肤粘膜瘀斑等血瘀证的表现,血瘀的程度与其股骨近端骨密度、甲皱微循环综合评分、血液流变学变化的变化有密切相关。3.老年人骨质疏松程度与其血瘀程度呈正相关性,表明血瘀是老年性骨质疏松症发病的一个病机。

【Abstract】 Objective:Based on osteoporosis patients’blood stasis "performance index of the macroscopic observation and analysis, senile osteoporosis, blood stasis exists for blood to change by method of treating senile osteoporosis, providing theoretical basis.Methods:On 30 cases senile osteoporosis,30 cases of patients with low density for "blood stasis" clinical manifestations of integrated assessment and wrinkly to knit, microcirculation and hemorheology detection, and testing of 30 cases as normal, healthy adults and blood stasis comprehensive evaluation and wrinkly to knit microcirculation score, hemorheology detection and analysis of related coefficient of bone.Results:1. Senile osteoporosis patients, senile low bone mass, healthy adults between the bone has a very significant difference (P<0.01),show the bone osteoporosis obviously lower than older age between youths, also have very significant difference.2.Senile osteoporosis patients’clinical performance rating and blood stasis "group compared with healthy adults, have very significant difference (P<0.01), Senile low bone mass groups "blood stasis" clinical performance rating group compared with healthy adults, have very significant difference (P<0.01),Senile osteoporosis patients’clinical performance rating and blood stasis"low bone mass groups with alzheimer’s, have significant difference (P<0.05) that osteoporosis patients have a pain, varicose vein under the tongue is dark purple tongue, bruises, oral mucosa as dark skin bruises of blood stasis, its" blood stasis "clinical performance rating and osteoporosis degree of parturients.3.Senile osteoporosis patients wrinkly to knit microcirculation comprehensive rating group compared with healthy adults, have very significant difference (P<0.01), Senile low bone mass groups with comprehensive rating wrinkly to knit microcirculation, healthy adults group compared with significant difference (P<0.05). Senile osteoporosis patients with comprehensive rating wrinkly to knit microcirculation low bone mass groups, amd is very significant difference (P<0.01), the elderly patients with osteoporosis microcirculation comprehensive pointrating wrinkly to knit with osteoporosis degree of parturients.4. Senile osteoporosis patients hemorheology indices and healthy adults, have very significant group compared difference (P<0.01), Senile low bone mass groups with hemorheology indices, healthy adults group compared with significant difference (P< 0.05). Senile osteoporosis patients hemorheology indices and senile low bone mass groups have very significant difference (P<0.01)or significant difference (P<0.05) that osteoporosis patients exist the hemorrheologic changes of blood rheology and the index and osteoporosis degree of parturients.Conclusion:1.Senile osteoporosis belong to TCM bones paralysis","GuBi" category, is associated with human aging and disease, senile osteoporosis patients because of its physiological pathology process, blood stasis on its incidence plays an important role.2.Senile osteoporosis patients have existed for varicose vein, under the tongue, tongue are dark purple lips bruises, dark skin bruises mucosal blood stasis etc, blood stasis degree and proximal femoral BMD,wrinkly to knit microcirculation comprehensive rating, the hemorrheologic changes are closely related to the change.3.People with osteoporosis degree of blood stasis degree of parturients, blood stasis is that the pathogenesis of senile osteoporosis, a pathogenesis.
