【摘要】 目的:评价国产封堵器治疗房间隔缺损(ASD)的疗效。方法:2000～2005年在我科使用国产封堵器治疗的ASD患者357例。术前及术后1周内、6个月、1、2、3年及以上行心电图、经胸超声心动图(TTE)等检查。结果:①手术成功率97.5%。并发症主要发生在术后早期,长期随访未见明显并发症。②ASD封堵术后右心容量负荷明显减少,左室容积及内径增大、左室收缩功能增强;长期随访右房、右室容积缩小,左室舒张末期前后径(LVDd),左室每搏量(LVSV)较术前明显增大,心脏形态与功能进一步改善。③其中38例多孔型房缺及复合畸形患者除1例双孔型ASD患者外手术皆成功(成功率97.4%)。结论:使用国产封堵器治疗继发孔型房间隔缺损适应范围广,成功率高,并发症少,安全可靠。更多还原

【Abstract】 Objective: To evaluate the efficiency of transcatheter closure of arial septal defect (ASD) with domestic-made occluder device. Methods: There were 357 patients with ASD accepted interventional therapy with domestic-made occluder devices between 2000 and 2005 in our department. They were assessed with electrocardiogram (ECG) and transtho-racic echocardiography (TTE) before the procedure and at follow-up of 1 week, six month, 1 year, 2 year, 3 year and more longer. Results: ① The success rate of the procedure was 97.5%. The complications mainly occurred in the early stage. During the follow-up, no serious complication was found. ②After transcatheter closure, the preload of right heart significantly decreased, while the volume and diameter of left ventricle increased and left ventricular systolic function improved . During the long-term follow-up, the volume of right atrium and right ventricle was decreased, left ventricular end-diastolic diameter (LVDd) and left ventricular stroke volume (LVSV) continued to increase. The cardiac morphology and function improved further. ③ Thirty-eight patients were associated with multifenestrated ASDs or complex malformations. Except one patient with two ASDs, all patients completed the procedure with a successful rate of 97.4%. Conclusion: The use of domestic-made occlucder device in the treatment of ASDs has a wide application range, high success rate and safe and reliable effect as well without significant complications.更多还原
