【摘要】 目的：通过观察老年下肢动脉硬化闭塞症患者的临床症状、踝肱指数、足背激光多普勒血流成像、甲襞微循环及血液流变学的变化，来探讨老年下肢动脉硬化闭塞症的微循环状态和前列腺素E₁脂微球载体制剂（Lipo-PGE₁，北京泰德制药有限公司生产）对下肢动脉硬化闭塞症患者的疗效。 方法：选择自2001年3月至2002年1月在301医院住院的36例下肢动脉硬化闭塞症患者（诊断标准符合1995年中国中西医结合学会周围血管疾病专业委员会制定的诊疗标准），均为男性，平均年龄68．5±4．5岁（60岁～93岁）。给予Lipo-PGE₁ 10微克加入100毫升生理盐水，静脉滴注，每日一次，共14天。观察治疗前后临床症状改善情况、踝肱指数变化、足背激光多普勒血流成像的改变、血液流变学、甲襞微循环指标（应用田氏甲襞微循环综合定量评分方法进行观察、测量、记分）、肝肾功能、血脂、血、尿常规等。统计方法采用Stata4．0统计分析软件，方差分析，t检验；p≤0.05判断为有显著差异。 结果；治疗后临床症状显著改善，甲襞微循环的总积分值明显降低（p<0．01），血粘度降低（p<0．05）。甘油三酯、胆固醇降低不明显（p>0．05），踝肱指数增加，足背激光多普勒血流成像明显改善．用药前后肝、肾功能及血、尿常规检查结果无显著差异（p>0．05）。用药过程中未发现明显毒副作用。 解放军军医进修学院硕士研究生论文 凯时治疗动脉硬化闭塞症疗效观察 结论：L i P。－PGE；可显著改善老年下肢动脉硬化闭塞症患者 的临床症状，降低血粘度，改善甲缓微循环，安全有效，无明显 毒副作用。有着广阔的应用前景。更多还原

【Abstract】 Objective: To investigate the effects of the Lipo-PGEi and the state of the microcirculation on the elderly patients with Low Extremity Atherosclerosis Disease (LEASD) by observing the alterations of clinical symptoms, ankle/ brachia index (ABI), Laser Doppler Perfusion Imagerofdorsum pedis ,nail fold microcirculation and hemorheology.Methods: 36 Male LEASD patients with mean age 68.5?, 5 years(60~93) were selected from inpatient of 301 hospital during March 2001 and January 2002. The alterations of clinical symptoms, ankle/ bsrachia index (ABI), Laser Doppler Perfusion Imager of dorsum pedis, nail fold microcirculation and hemorheological markers , liver and renal function, TG,TC,blood routine and urine routine were observed in the 36 patients of LEASD before and after treatment with Lipo-PGE,10 micrograms per day for 14days. Statistical analysis was performed using Stata 4.0 software.Result : After treatment , clinical symptoms, ankle/ brachia index (ABI), Laser Doppler Perfusion Imager of dorsum pedis were improvement significantly .The nail fold microcirculation total scores was decreased prominently (p<0. 01).the blood viscosity was decreased as well (p<0.05), ankle/brachia index (ABI), Laser Doppler Perfusion Imager of dorsum pedis were obviously improved , There were no significant differences in triglyceride, Cholesterol, liver and renal functions, blood and urine routines before and after the treatment(p>0. 05).There was no significant side effect during drug therapy.Conclusion: The Lipo-PGEi could improve the clinical symptoms , the nai fold microcirculation and decrease blood viscosity of the patients with LEASD .
