“缪刺法”对佐剂性关节炎大鼠的镇痛效应及机制研究

【摘要】 目的:“缪刺法”是《黄帝内经》中记载的一种针刺取穴方法,当今针灸临床仍有较广泛应用,其效应及机制有待阐明。本实验拟探讨“缪刺法”对佐剂性关节炎大鼠的镇痛效应及部分中枢镇痛机制。方法:将SD大鼠50只随机分为5组分别是空白组、模型对照组、针刺右后肢组、针刺左后肢组、针刺右前肢组,每组10只,以弗氏完全佐剂诱导的关节炎大鼠为急性炎症痛模型,以电针作为针刺方法,选取右后肢“足三里”、“涌泉”穴为正常取穴组的选穴、选取左后肢“足三里”、“涌泉”穴为左右缪刺组的选穴,选取右前肢“曲池”、“合谷”为上下缪刺组的选穴,选用热辐射、放射免疫分析等检测方法,测量局部痛阈及下丘脑内β-内啡肽(β-EP)及促肾上腺皮质激素(ACTH)、脊髓中强啡肽(Dynal)等生化指标,比较观察不同缪刺取穴法与正常取穴法的镇痛效果。结果:常规取穴法与缪刺法对疼痛模型大鼠均有良好的镇痛效应,两种缪刺取穴法比较,以“左右缪刺”取穴法为优。正常针刺法与左右缪刺组中炎症痛模型大鼠下丘脑中β-EP、ACTH及脊髓中Dynal的含量下降,上下缪刺法组只出现β-EP、ACTH含量的下降,未见脊髓中Dynal含量与对照组的统计学差异。结论:正常针刺、左右缪刺及上下缪刺法均对佐剂性关节炎大鼠均有较好的镇痛效应,以左右缪刺法为优,其部分作用机制可能是通过激发中枢神经系统中镇痛物质而产生的调节功能。

【Abstract】 Objective: Crossing selection of acupionts method is one of the acupoint-selecton methods recorded in HuangDi internal classic and is still applied in acupuncture clinic today, which clinical effect and mechanism are still need to be elucidated. This study is to discuss the effect and part of center mechanism of analgesia on adjuvant-induced arthritis rats of this method.Methods: A total of 50 SD rats were randomized into 5 groups , they are normal group, model comparison group, right-hind points group, left-hind points group, left-fore points group, with 10 animals in each group. Acute inflammation pain model was established by injecting Complete Freund’s adjunvant, taking electroacupuncture as stimulating technique, selecting“Zusanli”and“Yongquan”at right-hind limb as pricking position for model comparison group, selecting“Zusanli”and“Yongquan”at left-hind limb for left-right crossing prick group, selecting“Quchi”and“Hegu”at right-fore limb for upper-lower crossing prick group, heating radiation was used to measure local pain threshold, radioimmunoassay to measureβ-EP , ACTH in hypothalamus and Dynal in medulla so as to observe the analgesia effect of different crossing acupoint selecting methods from usually used one.Results: Better analgesic effect has been observed in Both normal acuponits selection group and crossing acupoints selection group. Compare two kinds of crossing acupoints selection method, the left-right one is better. Content ofβ-EP, ACTH and Dynal decrease in normal acupoint selection method group and left-right crossing acupoints selection method group in rats. Content ofβ- EP, ACTH decrease in upper-lower crossing acupoints selection group but there’s no obvious difference about Dynal content between this group and model group.Conclusion: Better analgesic effect of all of normal acupoints selection method, left-right crossing selection of acupoints method and upper-lower crossing selection of acupoints method has been abserved in the study, and the left-right crossing selection of acupoints method is the best. Parts of the effective mechanism is probably stimulating the regulation function of analgesic material in central nervous system.
