安冲汤止血调经作用机理的实验研究

【副题名】安冲汤对大鼠雌孕激素及其受体、子宫内膜血管形态及ET-1、iNOS的影响

【摘要】 目的:探讨《医学衷中参西录》安冲汤止血调经的作用机理,为古方今用以治疗月经过多、经期延长疗效机理提供实验研究依据。方法:采用同一条件喂养下的健康、成年、雌性、未孕SD大鼠,4-6月龄,随机设安冲汤高、中、低剂量组、丹贞颗粒组(中药阳性对照组)、空白对照组,连续灌胃给药15天后,检测大鼠股动脉血中雌二醇(E2)和孕酮(P)含量;HE染色观察大鼠子宫内膜血管数目及形态学的变化,包括血管面积、周长、最大直径及最小直径;免疫组化法检测大鼠子宫内膜雌激素受体(ER)及孕激素受体(PR),检测子宫局部血管活性物质——内皮素(ET-1)、一氧化氮合酶(iNOS)。结果:生殖内分泌:与空白对照组比较,安冲汤各剂量组及丹贞颗粒组雌激素水平有升高趋势,低剂量组及丹贞颗粒组与空白对照组比较,差异有显著性(P<0.05);与空白对照组比较,安冲汤各剂量组及丹贞颗粒组孕激素水平有升高趋势,但差异无显著性(P>0.05);与空白对照组比较,安冲汤各剂量组及丹贞颗粒组降低雌激素受体(ER)水平,安冲汤各剂量组与空白对照组比较,差异有显著性(P<0.05);与空白对照组比较,安冲汤各剂量组及丹贞颗粒组升高子宫内膜上皮孕激素受体(PR)水平,且差异有显著性(P<0.05);与空白对照组比较,安冲汤各剂量组升高子宫内膜间质孕激素受体(PR)水平,且差异有显著性(P<0.05):与空白对照组比较,安冲汤各剂量组升高子宫内膜孕激素受体(PR)总量水平,差异有显著性(P<0.05)。子宫内膜血管:与空白对照组比较,安冲汤各剂量组有降低子宫内膜血管数目和减小血管面积的趋势,但差异无显著性(P>0.05);从血管形态上看,与空白对照组比较,安冲汤各剂量组有减小子宫内膜血管最大直径趋势,但差异无显著性(P>0.05)。与空白对照组比较,安冲汤各剂量组可减小子宫内膜血管周长及最小直径,且差异有显著性(P<0.05)。子宫内膜局部血管活性物质:与空白对照组比较,安冲汤各剂量组及丹贞颗粒组能升高子宫内膜内皮素(ET-1)水平,且差异有显著性(P<0.05);与空白对照组比较,安冲汤各剂量组及丹贞颗粒组降低子宫内膜-氧化氮合酶(iNOS)水平,且差异有显著性(P<0.05);结论:1、生殖内分泌:安冲汤可能通过升高血清中雌、孕激素含量,降低子宫内膜雌激素受体含量,升高孕激素受体含量,起到对子宫内膜的调节作用,而起到调经止血作用。2、子宫内膜局部血管:安冲汤可以通过降低子宫内膜血管数目、面积及血管形态的改变,以及降低子宫内膜iNOS水平,升高子宫内膜内皮素(ET-1)水平,影响子宫内膜血管舒缩功能,促进子宫内膜血管收缩,从而达到止血调经的目的。

【Abstract】 OBJECTIVE:An-Chong-Tang(ACT)comes from《Yi Xue Zhong Zhong Can Xi Lu》.In order to provide the theory and experiment gists for the use of ACT,the experiment explore the mechanism of ACTon arresting and regulating menstruation.METHODS:Healthy female SD mice which were 4-6 months,had same growing conditions,had grown up and had’t gravid,were randomly devided into five groups.They are higher, Middle and lower dosage groups of ACT,Dan Zhen Ke Li group and normal group. Medicine were continually given to the mice in fifteen days Observation were assoc-iated with effect to the mice of Progesterone(P),Estradio(E2),endometrium estrogen receptor(ER),endometrium progesterone receptor(PR),amount and variance of morph of endometrial blood vessel,endothelin-1(ET-1)and nitricoxide symthase(iNOS).RESULT:1.On geno-endocrine:Compared with normal group,E2 of each group of ACT and Dan Zhen Ke Li group are higher.Lower dosage group of ACT and Dan Zhen Ke Li group are significant(P<0.05).P of each group of ACT and Dan Zhen Ke Li group have higher tendency,but there are no statistical difference (P>0.05).Compared with normal group,ER of each group of ACT and Dan Zhen Ke Li group are lower,and there are significant(P<0.05).Compared with normal group,PR of each group of ACT and Dan Zhen Ke Li group are higher,and there are significant(P<0.05).2.Observation of endometrial blood vessel:Compared with normal group,amount and area of endometrial blood vessel have lower tendency,but there are no statistical difference(P>0.05).Compared with normal group, each group of ACT can decrease the perimeter and minimum diameter of endometrial blood vessel,and there are significant(P<0.05). 3.On vasoactive substance of endometrial blood vessel:Compared with normal group,each group of ACT and Dan Zhen Ke Li group can rise up the express of ET-l,and there are significant difference(P<0.05),Compared with normal group,each group of ACT and Dan Zhen Ke Li group can degrade the express of iNOS,and the-re are significant(P<0.05).CONCLUSIONS:1.On gen-endocrine:ACT can effect the endometrium by change the express of E2. P,PR and ER.Then may arrest and regulate menstruation.2.On endometrial blood vessel and vasoactive substance of endometrial blood vessel:ACT may decrease amount area of endometrial blood vessel,change the shape of endometrial blood vessel,rise up the express of ET-l,and degrade the express of iNOS,then may effect the function of relaxation and contraction to arrest bleeding and regulate menstruation.
