饮水型砷中毒对人体红细胞膜结构形态影响的研究

【摘要】 目的:探讨饮水型砷中毒对人体红细胞膜结构形态的影响。方法:对地方性砷中毒病区45例地砷病患者(病例组),病区非地砷病患者87例(内对照组),邻近村非病区健康人58例(外对照组)抽取抗凝血,分离血清,采用分光光度法测定各组红细胞膜Na+,K+-ATP酶活性,高效液相法测定各组红细胞膜磷脂组分(磷脂酰胆碱PC;磷脂酰乙醇胺PE;磷脂酰丝氨酸PS;神经鞘磷脂SM)的含量,使用扫描电镜观察各组红细胞膜形态的改变,水、尿、发中砷含量的测定均采用二乙氨基二硫代甲酸银比色法。结果:1)外对照组、内对照组、病例组红细胞膜的PS、PE、PC的含量依次降低(P<0.05),病例组SM含量低于外对照组(P<0.05),内对照组SM与病例组和外对照组相比,差异无统计学义(P>0.05),SM/ PC比值病例组高于内对照组和外对照组(P<0.05),内对照组SM/ PC比值与外对照组相比,差异无统计学义(P>0.05),轻、中、重度砷中毒患者红细胞膜4种磷脂组分及SM/ PC比值的比较,差异无统计学意义(P>0.05);2)外对照组、内对照组、病例组Na+-K+-ATP酶活性依次降低(P<0.05),轻中重度地砷病患者Na+-K+-ATP酶活性比较,差异无统计学意义(P>0.05);3)红细胞膜Na+-K+-ATP酶活性与红细胞膜PS、PE、PC呈正相关关系(P<0.01),与红细胞膜SM无相关关系(P>0.05);4)扫描电镜观察内对照非地砷病患者和轻中重度病人的红细胞膜形态均发生了改变,且膜的受损程度与病人病情相一致;5)外对照组、内对照组及病例组尿砷含量比较,差异无统计学意义(P>0.05),轻中重度地砷病患者尿砷含量比较,差异无统计学意义(P>0.05);6)外对照组的健康人发砷含量要低于病例组患者及内对照组的非地砷病患者(P<0.05),病例组与内对照组发砷含量比较,差异无统计学意义(P>0.05),轻、中、重度组患者发砷含量比较,差异无统计学意义(P>0.05);结论:饮水型砷中毒可导致人体红细胞膜结构形态的损伤。

【Abstract】 Objective:To explore the effects of arsenic poisoning through drinking water on erythrocyte membrance structure and shape in human. Methods: 45 arsenism patients from arsenism endemic areas (Case Group), 87 non-arsenism patients from prevailing areas (Inner-control Group) and 58 healthy persons from non-prevailing areas which is a neighboring village with the prevailing areas (Outer-control Group) were obtained. we acquired anticoagulant blood from above-mentioned persons and separated serum, The activity of Na+, K+-ATPase in several groups erythrocyte membrane were measured by spectrophotometric. The contents of phospholipids (PS、PE、PC、SM)in red cells membrane were detected by HPLC. Configuration of three groups erythrocyte membrane were observed by SEM. The concentrations of arsenic in water, urine and hair were detected with DDC. Results: 1) The contents of PS、PE、PC in red cells membrane reduce in turn in outer-control group, inner-control group, case group(P<0.05), SM in the case group is less than the outer-control group(P<0.05). SM is undifferentiated in inner-control group compared with the outer-control group and case group(P>0.05). SM/PC is higher in the case group than the inner-control group and the outer-control group(P<0.05),SM/PC is no difference between the inner-control group and the outer-control group(P>0.05).Four phospholipids and SM/PC are no difference in the light patients,moderate patients and severe patients.2)The activity of Na+, K+-ATPase in red cells membrane reduce in turn in outer-control group, inner-control group, case group(P<0.05). Na+, K+-ATPase is no difference in the light patients,moderate patients and severe patients(P>0.05). 3)There is a positive correlation between Na+, K+-ATPase and PS, PE, PC(P<0.01).The Na+, K+-ATPase is no correlation with SM. 4)Configuration of erythrocyte membrance have deformed from internal control residents to different degree of patients by SEM. Besides,there are in good line between the extent of damage to erythrocyte membrance and condition of patients. 5)Arsenic content in urine is nodifference in outer-control group, inner-control group, case group(P>0.05). This result is also found in different degrees of patients(P>0.05). 6)Hair arsenic contents in outer-control residents is lower than patients and inner -control residents(P<0.05). Hair arsenic contents is no difference between inner-control residents and patients(P>0.05). Hair arsenic contents is no difference in outer-control group, inner-control group, case group(P>0.05).Conclusions: Arsenic poisoning through drinking water can damage erythrocyte membrance structure and shape in human.
