胎儿生长受限患者MMP-9水平的变化及其意义

【摘要】 目的:通过检测胎儿生长受限((fetal growth restriction,FGR)孕妇的肘静脉血及其分娩的新生儿脐静脉血中基质金属蛋白酶-9(matrix metalloproteinases-9,MMP-9)的水平,探讨MMP-9在胎儿生长受限发病中的作用。方法:采用固相夹心酶联免疫吸附法(ELISA法)分别检测20例胎儿生长受限孕妇的肘静脉血及其新生儿脐静脉血中MMP-9的水平(FGR组)、20例正常妊娠足月孕妇的肘静脉血及其新生儿脐静脉血(正常组)中MMP-9的水平。所得数据用SPSS13.0统计软件包处理。结果:(1)FGR组孕妇肘静脉血MMP-9水平为(103.00±38.71)ng/ml,正常组孕妇的肘静脉血MMP-9水平为(196.60±78.91)ng/ml,FGR组孕妇肘静脉血MMP-9水平低于正常组,两组比较差异具有显著性(P<0.001)。(2)FGR组新生儿的脐静脉血MMP-9水平为(80.30±30.47)ng/ml,正常组新生儿的脐静脉血MMP-9水平为(153.80±83.62)ng/ml,FGR组新生儿脐静脉血MMP-9水平低于正常组,两组比较差异具有统计学意义(P<0.05)。(3)FGR组、正常组中孕妇肘静脉血中的MMP-9水平均高于新生儿脐静脉血中的MMP-9水平(P<0.05,P<0.05),且FGR组、正常组中孕妇肘静脉血中的MMP-9水平均与新生儿脐静脉血中的MMP-9水平呈正相关关系(r=0.593,r=0.624)。(4)FGR组中孕妇肘静脉血和新生儿脐静脉血中的MMP-9水平均与新生儿体重呈正相关关系(r=0.476,P<0.05;r=0.567,P<0.05),正常组中孕妇肘静脉血和新生儿脐静脉血中的MMP-9水平均与新生儿体重无相关关系(P>0.05,P>0.05)。结论:胎儿生长受限孕妇的肘静脉血及其新生儿脐血中的MMP-9水平均明显下降,MMP-9水平与胎儿生长受限新生儿的出生体重呈正相关关系,而与正常新生儿的出生体重无相关关系,MMP-9在胎儿生长受限的发生、发展中起重要作用,有望作为预测胎儿生长受限的新指标之一。

【Abstract】 Objective:To investigate the relationship between matrix metalloproteinases-9 (MMP-9) and fetal growth restriction,we detected the concentration of MMP-9 in elbow vein blood from pregnant women with fetal growth restriction (FGR) and umbilical vein blood from their newborns.Methods:Twenty pregnant women of fetal growth restriction and their newbomes (the FGR group),twenty normal pregnant women in term and their newbomes (the normal group)were studied.The concentration of MMP-9 in maternal elbow vein blood and umbilical vein blood were determined by solid phase sandwich enzyme linked immuno-sorbent assay(ELISA).These experimental data was analized by the software SPSS13.0.Results:(1) The level of MMP-9 in maternal elbow vein blood of FGR group was(103.00±38.71)ng/ml,the level of MMP-9 in maternal elbow vein blood of the nomal group was(196.60±78.91 ) ng/ml,the level of FGR group is lower than the nomal group,the two groups had statistically significant deviation(P<0.001).(2) The level of MMP-9 in umbilical vein blood of FGR group was(80.30±30.47) ng/ml,the level of MMP-9 in umbilical vein blood of the nomal group was(153.80±83.62) ng/ml,the level of FGR group is lower than the nomal group,the two groups had statistically significant deviation(P<0.05).(3) The levels of MMP-9 in maternal elbow vein blood were higher than the levels of MMP-9 in umbilical vein blood in both of the the FGR groups and the nomal groups(p<0.05,p<0.05),and the levels of MMP-9 in maternal elbow vein blood were positive correlated with the levels of MMP-9 in umbilical vein blood in both of the FGR groups and the nomal groups(r=0.593,r=0.624).(4) The levels of MMP-9 in both of maternal elbow vein blood and umbilical vein blood were positive correlated with the birth weight of newbomes in the FGR groups(r=0.476,P<0.05;r=0.567,P<0.05),the levels of MMP-9 in both of maternal elbow vein blood and umbilical vein blood was not correlated with the birth weight of newbornes in the nomal groups(P>0.05,P>0.05)Conelution:It showed that the significantly reduced levels of MMP-9 in both in maternal elbow vein blood and umbilical vein blood of fetal growth restriction,and the levels of MMP-9 were positive correlated with the birth weight of newbomes in the FGR groups,but the levels of MMP-9 was not correlated with the birth weight of newbornes in the nomal groups, MMP-9 had improtant actions in occurrence and development of FGR,it may be one of the new indicater of predicting FGR.
