盐酸戊乙奎醚联合氯解磷定治疗混配农药中毒的实验研究

【摘要】 背景上世纪90年代以来,为克服害虫抗药性,提高杀虫药效,混配农药逐渐占据我国农药市场,这也是我国当前和今后十年农药发展的主要趋势,而混配农药中毒的病例也随之大量增多。随着农药混剂的大量生产和广泛应用,使我国混配农药中毒病例不断上升,混配农药的中毒问题日益突出。盐酸戊乙奎醚注射液(长托宁),其化学名称为3-(2-环戊基-2羟基-2苯基-乙氧基)奎宁环烷盐酸盐,是中国军事医学科学院毒物药物研究所设计合成的新型抗胆碱能药物,2003年被批准为“卫生部面向农村和基层推广适宜技术十年百项计划”项目(盐酸戊乙奎醚取代阿托品救治有机磷农药中毒技术)。其主要作用特点,不但抗胆碱作用强,而且作用全面、持续时间长。同时由于其对M受体亚型具有高度选择性,毒副作用较小,被广泛应用于麻醉前给药、对抗有机磷的毒性作用等。近年来有关盐酸戊乙奎醚基础研究和临床应用进展迅速,然而,有关盐酸戊乙奎醚治疗混配农药中毒的疗效研究却未见报道,本文就此进行了深入细致的实验研究,为进一步临床应用提供了理论依据。目的观察盐酸戊乙奎醚注射液(长托宁)联合氯解磷定治疗混配农药中毒的治疗效果方法选取成年健康雄性Wistar大鼠随机分为对照组、染毒非治疗组、6h治疗组、12h治疗组和24h治疗组,实验结束后留取动物血清冻存待检测。结果染毒非治疗组大鼠病死率为100%,治疗组大鼠全部存活,差异非常显著(P<0.01)。染毒非治疗组大鼠血清BUN、Cr、ALT、AST、ALP、GLU较对照组明显升高(P<0.01),而治疗组各时段大鼠血清BUN、Cr、ALT、AST、ALP、GLU明显低于染毒非治疗组(P<0.01)。染毒非治疗组与治疗组各时段大鼠血清TP、ALB、A/G较对照组明显降低(P<0.01)。染毒非治疗组大鼠血清CHE活力较对照组明显降低(P<0.01)。而治疗组各时段大鼠血清CHE活力明显高于染毒非治疗组(P<0.01)。治疗组各时段大鼠血清CHE活力明显低于对照组(P<0.01)。染毒非治疗组大鼠血清TNF-α水平较对照组明显升高(P<0.01)。而治疗组各时段大鼠血清TNF-α水平明显低于染毒非治疗组(P<0.01)。结论:长托宁联合氯解磷定治疗混配农药中毒初步显示具有较好的疗效。

【Abstract】 Objective To observe the treatment effect of penehyclidine hydrochloride combined with pralidoxime chloride on mixed agricultural chemicals poisoning.Methods All 50 healthy adult Wistar rats were randomizedly assigned into normal control groups,paraquat group,6h treatment group,12h treatment group and 24h treatment group.Serum was obtained at the end of experiment.Serum BUN,Cr,ALT, AST,ALP,TP,ALB,GLB,A/G,GLU and CHE was detected by automatic biochemistry analyzer and serum TNF-αwas detected by ELISA.Results The paraquat group animal were died within six hours.None animal died in treatment group and control group.The serum BUN,Cr,ALT,AST,ALP,GLU of paraquat were higher than that of control groups(P<0.01).The serum BUN,Cr,ALT, AST,ALP,GLU of treatment groups was lower than that of control groups(P<0.01)。The serum CHE of paraquat groups was lower than that of control groups(P<0.01)。The serum CHE of treatment groups was higher than that of paraquat groups (P<0.01)。The serum CHE of treatment groups was lower than that of control groups(P<0.01).The serum TNF-αof paraquat groups was higher than that of control groups(P<0.01).The serum TNF-αof treatment groups was lower than that of paraquat groups(P<0.01) Conclusion The treatment of penehyclidine hydrochloride combined with pralidoxime chloride is effective on mixed agricultural chemicals poisoning.
