参附注射液不同剂量治疗缓慢性心律失常心阳虚证疗效的临床观察

【摘要】 缓慢性心律失常是心血管系统的常见病之一，西医治疗缺乏理想的药物，而安装人工心脏起搏器尚未得到临床普及。大量临床试验表明，参附注射液治疗缓慢性心律失常有较好疗效。本课题主要观察和总结两种剂量的参附注射液治疗缓慢性心律失常心阳虚证的效果，为更合适使用参附注射液提供依据，丰富缓慢性心律失常的中医治疗方案。论文包括文献综述和临床研究两个部分。文献综述包括两篇。第一篇为《中医学对缓慢性心律失常的临床研究进展》，主要总结了中医学运用辨证论治、自拟方、经方成方、中成药和静脉制剂等治疗缓慢性心律失常方面的研究概况及进展。第二篇为“参附注射液的临床研究进展”，阐述了参附注射液的药理作用，并总结了参附注射液临床上治疗休克、心力衰竭、心律失常、冠心病等多种疾病的研究进展。临床研究部分，收集北京中医药大学附属东方医院急诊科及心内科60例缓慢性心律失常心阳虚病人，随机分为两组，分别给予50ml及100ml两种剂量的参附注射液治疗，观察治疗前后心率、心电图、心阳虚证候评分及主要中医症状的变化情况，经统计分析，比较两剂量组的疗效。结果显示，100ml参附注射液在改善心阳虚证主要症状及整体证侯上有明显优势，在急诊临床治疗缓慢性心律失常中具有一定的应用价值。更多还原

【Abstract】 Slow arrhythmia is one of the most common diseases in cardiovascularsystem. There isn’t ideal drug for modern medicine in treating this disease andthe pacemaker has not been widely used in clinical treatment. Large numberof clinical trials indicate that Shenfu injection has good curative effect intreating Slow arrhythmia. This study mainly observe and summarize the effectof two different doses Shenfu injections in treating Slow arrhythmia whichbelongs to“Deficiency of yang symptoms”in order to provide hereunder inproper using of the injection and enrich the treatments in slow arrhythmia forChinese medicine.Papers include two parts about literature overview and clinical research.There are two literature overviews in this part, the first is“the progress ofclinical research in Slow arrhythmia related to Chinese medicine”, whichmainly summarizes the general situation and progresses in treating Slowarrhythmia using patent formulas, classic formulas, nonclassicformulas andvenous injections. The second one is about the clinical research progress ofShenfu injection which illustrates the pharmacologic effect of the Shenfuinjection and summarizes the research progresses in treating shock, heartfailure, arrhythmia, coronary heart disease and others.Regarding the part of clinical research, all 60 cases of patients with Slowarrhymia deficiency of yang were collected from emergency department andcardiovascular department in Dongfang hospital, then they were randomizedinto two groups under treatment of 50ml dose and 100ml dose Shenfuinjection. The changes of heart rate, ECG and main symptoms in Chinesemedicine were observed and analyzed to compare the effect of two dosegroups. Results showed that 100ml dose has great advantage in improvingmain symptoms of“deficiency of yang”and overall Syndrome. As a result,100ml dose of Shenfu injection has great clinical significance in treating slowarrhythmia for emergency department.更多还原
