山东省综合性医院省医药卫生重点学科发展评价研究

【摘要】 山东省医药卫生重点学科是经过山东省卫生厅认定,代表本学科发展方向,反映山东省医学科学整体水平的学科,其水平是山东省医疗卫生学术水平的重要标志。为促进省医药卫生重点学科的发展,省卫生厅、财政厅及各所在单位都进行了不同程度的人力、物力、财力投入。本研究主要目标是了解综合性医院中的山东省医药卫生重点学科发展状况,探讨影响学科发展的主要因素;提出促进省医药卫生重点学科发展的政策建议,为重点学科管理提供参考。本研究采用目的抽样方法,抽取4所医院的12个重点学科进行调查研究。通过深入各单位调查和查阅有关档案资料,收集学科投入、产出等相关数据资料。采用EXCEL2000进行数据录入,选用SPSS13.0统计软件进行统计、分析。本研究的主要结果:(1)学科学术队伍进步显著。自2000年到2006年,学科人员总数从297人增长到354人,增长了19%;高级职称人员比例从50.5%增长到68.1%;硕士以上学位人员比例从48.5%增长到89%。(2)科研水平有所提高。2000-2006年各学科共获得国家级科研项目30项,省部级160项,厅局级102项。科研立项特别是国家级项目立项随年度明显增加。各学科共获得国家级奖励5项,省部级奖励60项,厅局级奖励108项;国家专利28项,主编、主译著作78部。共发表论文1445篇,其中SCI文章47篇,中华系列论文277篇,其他核心期刊论文1234。篇。在SCI论文发表上,年度之间进步明显,2000年发表2篇,2006年发表15篇,是2000年的7.5倍。(3)各学科培养人才逐渐增多。2000-2006年各学科共培养博士研究生175名,硕士研究生472名,进修生1463名。2006年培养博士比2000年增长175%;硕士增长63.5%,进修生增长40.5%。(4)学科临床工作在各医院中优势明显。各重点学科总体的床位使用率从2000年的89.17%上升到95.36%。2000年平均门诊人次546195人次,2006年达到629691人次,增长15.29%,占全院年门诊人次的比例从6.45%上升到7.28%。(5)各学科发展水平有差距,首批与第二批重点学科间差距不明显。(6)学术带头人水平和学术梯队素质是影响学科发展水平的首要因素,学科发展方向及发展规划、学科经费投入、学科管理体制等都是影响学科发展的主要因素。学科建设中主要存在五方面的问题:一是学科发展不平衡;二是缺乏重大的、有影响力的科研项目,国家级科研奖励少、获国家专利少,国内、国际合作与交流有待进一步加强;三是学科整体水平与全国先进学科水平还有差距;四是学科建设经费不足,学科经费的使用主要用于大型仪器设备的购置,在人才队伍建设上投入的经费比例相对较小;五是学科管理比较松散。政策建议:第一,进一步明确学科发展方向,制定科学合理的学科发展规划。第二,加强学科人才梯队建设,特别要加强中青年人才的培养。第三,加强临床科研,特别是高层次临床科研。第四,建立切实可行的激励约束机制。第五,增加学科投入,集中有限的资金发展优势学科。

【Abstract】 The key medical and health disciplines of Shandong Province which have been confirmed by Shandong Provincial Health Department are on behalf of the disciplines’ development directions, and their levels are an important indicator of academic standards of Shandong Province’s medical and health. In order to promote their development, Shandong Provincial Health Department, Shandong Finance Department and other related departments have put varying degrees of manpower, material and financial resources.The purpose is to understand the key disciplines’ development status of general hospitals in Shandong province, to explore the main factors which effect the development of academic disciplines, to raise policy recommendations which can promote the development of key disciplines in the medical and health, and to provide reference for the management of key disciplines.This study uses the Purpose sampling method,to conduct a survey of key disciplines through extracting four of the twelve hospitals.Through in-depth investigating the hospitals and consulting archival materials,the disciplines inputs, outputs, and other related data sources are collected.Data entry is by Excel 2000,and statistical analysis is use of statistical software SPSS13.0.The study showes: 1.The academic ranks have had significant progress. 2. The level of scientific research has improved. 3. The personnel training of every Subject has increased gradually. 4. The clinical work of key Subjects in each hospital has obvious advantages. 5. There is a gap between the level of development of various disciplines, but the first and second batch of key desciplines have no obvious gap. 6. The development directions of academic disciplines, the academic echelon level and the input of funds have a great effect on the level of scientific research.There are five sorts of problems in disciplines’ building. Firstly, the disciplines’ development is uneven; Secondly, there are lack of major research projects which may have great effects, and there are few projects which can bear the national research awards and the national patents, furthermore,the cooperation and exchanges among countries should be further strengthened; Thirdly, the whole disciplines’ level has a great gap compared with the national advanced disciplines’; Fourthly, the subjects’ funds is deficient, and the majority of the funds are used to buy large apparatuses and equipments, so the outlay devoted to the manpower building is in a small proporation; Fifthly, the management of the desciplines is relatively loose.First, the development directions of the academic disciplines should be further defined, and the scientific and rational development planning should be made; Second, the disciplines’ echelon talents , especially the young or middle-aged talents, should be brought up strengtheningly;Third, the clinical research ,especially high-level clinical research , should be enhanced; Fourth, the practical incentive and restriction mechanisms should be established; Fifth, the government should increase investment to the desciplines, and concentrate limited funds to develop preponderant disciplines.
