乳腺肿瘤三维超声血管容积指数与病理微血管密度相关性研究

【摘要】 目的 肿瘤病理微血管密度是评价肿瘤血管生成或血管丰富程度较为客观的指标,也是预测乳腺癌预后的重要指标。多普勒超声技术可以显示小动脉、小静脉的低速血流信号,可以间接评价肿瘤血管生成活性或肿瘤内血流信号丰富程度。已有作者应用彩色多普勒技术分别测定肿瘤二维彩色血流分级、彩色像素密度或三维超声血管指数(半定量),来评价肿瘤内血管生成,并分别与病理微血管密度做相关性分析,研究认为二维CPD、三维血管指数与MVD均有一定的相关性。但是对于三维超声另一新的定量指标“血管容积指数”的研究及其与肿瘤病理MVD相关性的研究尚鲜有报道。本文应用Tomtec 3D Sono Scan Pro1.2对乳腺肿瘤进行三维重建,定量计算肿瘤的三维血管容积指数,同时计算机辅助测量肿瘤血流的二维彩色像素密度CPD,并对手术后的标本进行抗CD34因子免疫组化染色,测定肿瘤病理微血管密度,目的在于对比分析CPD,VVI与MVD的相关性,探讨VVI在评价乳腺肿瘤血管生成中的应用价值。 方法 应用GE Logiq700型彩色超声诊断仪,探头频率为13MHz,对42例乳腺肿瘤进行二维彩色多普勒超声检查,同时采用Tomtec 3D Sono Scan Pro1.2工作站对乳腺肿瘤进行三维重建,计算肿瘤的三维血管容积指数,计算机辅助测量肿瘤血流的二维彩色像素密度,对手术后的标本进行抗CD34因子免疫组化染色,测定肿瘤微血管密度。采用偏相关分析法分析乳腺肿瘤的二维彩色像素密度、三维血管容积指数与微血管密度相关性。

【Abstract】 ObjectiveAngiogenesis is the formation of new capillaries from the existing vascular network and is essential for all tumor growth and dissemination. Some studies have shown that intratumoral microvessel density (MVD) is an important prognostic marker of survival in breast cancer. Although there have been reports on the correlation between Doppler sonography and pathologic MVD, but there are no studies in three-dimensional ultrasound vascularity volume index(VVI).The aim of this study was to determine the relationship between three-dimensional ultrasound VVI or two dimensional color pixel density(CPD) and pathologic microvessel density(MVD) of breast masses.Methods42 patients with breast masses were examined by three-dimensional ultrasound(3D-US), calculate the three dimensional VVI of breast masses. Computer assisted analyzed two dimensional color pixel density of breast masses. MVD was calculated in surgical specimens after immunohistochemical staining using anti-factor CD34.Results3D-US provided a clear stereoscopic view of breast masses. These studies showed a significant linear correlation between CPD/VVI and MVD(rCPD=0.4318, P=0.005;
