RhBmp-2、聚乳酸复合体植入犬拔牙创促进牙槽骨再生的实验研究

【摘要】 [目的]:通过将rhBmp-2与聚乳酸的复合体植入犬拔牙创内,利用X线检查、光镜、扫描电镜观察实验组拔牙创与对照侧拔牙创牙槽骨的愈合情况并做对比,为临床寻找一种适合实际应用的促进牙槽骨再生的新型的生物材料。[方法]:选成年雄性牙周健康杂种犬6只,随即方法分为2组,每只犬全麻下间隔拔除左右双侧上下颌前磨牙,两侧相同牙位做自体对照,一侧拔牙创内充填rhBmp-2与聚乳酸的复合体,缝合创口,一侧缝合创口,自然愈合做对照,于2周后,6周和10周行X线检查,于6周和10周分别处死2组犬,取标本行光镜和扫描电镜观察成骨情况,材料与新生骨结合情况。[结果]:复合体材料充填组,术后6周拔牙创处出现较清晰骨小梁影像,与周围松质骨牙槽骨影像相近,对照组拔牙创骨小梁影像密度则低于周围牙槽骨影像。术后10周,实验组骨小梁影像密度明显高于周围松质骨牙槽骨影像。对照组骨小梁影像与周围牙槽骨影响相近。病理观察实验组6周生成的骨小梁比对照组粗大成熟度高,扫描电镜观察术后6周材料与新生骨结合紧密,结合面为骨性结合,材料内有新生毛细血管,成骨活跃,术后10周,实验组已生成较为成熟骨组织,骨陷窝缩小,材料基本降解。[结论]:rhBmp-2、聚乳酸复合体是一种促牙槽骨再生的新型生物复合材料,并可缩小拔牙创,减少出中文摘要血,促进拔牙创的愈合,有希望成为临床实际应用的修复牙槽骨缺损的新型材料。

【Abstract】 Objective To find a new practical material which can improve the regeneration of alveolar bone by filling rhBmp-2/polylacid composite in extraction soket of dog. Methods rhBmp-2/ploylacid composite was implanted in extraction socket, as was compared with normal healing extraction socket, x-ray evaluation and histological observation and scanning eletronic microscope examination were used to evaluate composite’s effects on inducing the regeneration of alveolar bone. Result After 6 weeks, in groups rhBmp/polylacid, bulk bone could be obviously observed ,its image density was close to the bone around extraction soket and higher than control groups., x-ray evaluation and histological observation found extraction sockets filled with composite healed better and more bone was regenerated than control side. After 10 weeks , in groups rhBmp/polylacid, image density of bulk bone was higher than that of bone around extraction soket.Histological observation found extraction sockets filled composite healed better and more bone was regenerated than control side, scanning eletronic microscope examination found that after 6weeks , composite jointed closely with regenerated bone and they had bone interface, after 10 weeks regenerated bone matured, composite was biodegraded. Conclusion rhBmp-2/ploylacid composite can serve as a new biological material in inducing the regeneration of alveolar bone and improving the healing of extraction socket.
