【摘要】 本文运用传统的中医思维方式，从理论、临床和实验三方面对慢性胆囊炎的病因病机和治则治法进行深入的探讨，以寻求治疗慢性胆囊炎的有效方药。本文提出慢性胆囊炎肝郁气滞、脾气虚弱，兼夹瘀血、湿热的病机假说，拟定疏肝健脾、利胆祛湿、清热通经止痛之法，据此遣药组方，治疗慢性胆囊炎肝郁脾虚证患者46例，并与40例消炎利胆片对照组作比较研究。两组均治疗2个疗程(2个月)，对症状、体征、舌象、脉象、胆囊B超影像学等进行了综合评定。 结果表明，香青利胆煎试验组临床疗效显效率73.91％，总有效率89.13％，明显优于消炎利胆片对照组(P＜0.05)；腹部B超影像学检查表明，胆囊形态及其收缩功能改善情况明显优于对照组；动物实验表明，香青利胆煎具有促进胆汁分泌、增加胆汁流量、止痛、抗炎等作用。 结果证明，肝郁气滞、脾气虚弱，瘀血湿热互结是慢性胆囊炎肝郁脾虚证的中药病机，疏肝健脾，利胆祛湿，清热通经止痛是慢性胆囊炎的重要治法，香青利胆煎治疗慢性胆囊炎肝郁脾虚证不仅具有显著的临床疗效，而且具有坚实的药理药效学基础，证明该药是治疗慢性胆囊炎的有效复方。本文从理论上深化了中医学对慢性胆囊炎的病机认识，对该病的治疗提出了新的研究思路和治疗方药，为丰富该病的治疗学做出了有益的尝试。更多还原

【Abstract】 To search after the effective medicine to Chronic Cholecystitis, the article, on the base of the traditional Chinese medicine, theoretically, clinically and experimentally discusses the cause and the mechanism of the disease, and the treatment of the disease. The article advances a new mechanism of Chronic Cholecystitis that the disease is mainly caused by liver depressed and spleen weakness plus extravasated blood and dampness. According to this mechanism,a new prescription of Xiangqinglidanjian Decoction had been drawn up to treat 46 patients. A Clinical Observation had been carried, the treatment group (46 cases) with Xiangqinglidan Decoction and contract group (40cases) with LidanPian were set up randomly. The two treatments both being taken for two months (one course lasting for one months), have a comprehensive analysis of the effects through the symptom,body sign, tongue condition, pulse condition and gallbladder condition by "B" machine.The result turns out as followings: The treatment group had 73.91% in clinical complete alleviated and 89.13% in total effective rate, superior to contrast group (P<0.05) and the stomach reflection showing distinct improvement. The animal experiment clearly showed that Xiangqinglidan Decoction has the effects on improvement of the bile secretion, increase of bile discharge, improvement of bile system’ microcirculation and anti-inflammation etc.Then the conclusion clearly shows that the mechanism of Chronic Cholecystitis ismainly caused by liver depressed and spleen weakness plus extravasated blood and dampness. To relax the depressed liver and invigorate the function of the spleen, improve the function of the gall and relieve the dampness is the important way of curing Chronic Cholecystitis. Xiangjianlidan Decoction has obvious clinical effect to treating Chronic Cholecystitis and the effect has been confirmed on the base of Pharmacology and Materia medica.The article theoretically improves Chinese Medicine’ understanding to the Mechanism of Chronic Cholecystitis and offers a new study method.
