【摘要】 目的：观察清金化痰汤配合穴位注射疗法对痰热壅肺型支气管扩张急性加重期患者的临床症状、体征、炎症指标等的影响,为临床运用中医疗法治疗支气管扩张提供理论依据。方法：把30例支气管扩张急性加重期辩证属痰热壅肺型的患者,随机分为治疗组和对照组两组。对照组用单纯西医常规治疗,治疗组在西医常规治疗基础上加用清金化痰汤配合穴位注射疗法治疗,疗程为10天。观察两组在治疗前后临床症状、体征、炎症指标等的变化。结果：1.清金化痰汤配合穴位注射疗法能改善支气管扩张患者临床证候本研究结果显示,两组治疗前后咳嗽、咯痰、咯血、发热、喘息、胸痛、湿啰音等各项主要症状及体征相比较,差异具有显著性(P<0.01),说明两组的治疗方案可明显改善患者的症状与体征。治疗组治疗后各项主要症状及体征与对照组相比较,差异具有显著性(P<0.05或P<0.01),提示治疗组的治疗方案在改善患者的症状与体征方面优于对照组。从证候积分综合评价,治疗组的总有效率为93.3%,对照组的总有效率为80.0%,治疗组疗效优于对照组,具有显著性差异(P<0.05)。2.清金化痰汤配合穴位注射疗法能有效降低支气管扩张患者炎症指标本研究结果显示,两组治疗前后炎症指标CRP与WBC相比较,具有显著性差异(P<0.01),提示两组治疗方案均可降低机体炎症指标,有效控制支气管-肺组织的感染；治疗组治疗后CRP、WBC与对照组相比较,亦有显著性差异(P<0.05),说明治疗组在降低机体炎症指标及控制支气管-肺组织的感染方面优于对照组。结论：采用清金化痰汤配合穴位注射疗法治疗支气管扩张急性加重期患者,能明显改善患者的临床症状和体征,取得良好的临床疗效；清金化痰汤配合穴位注射疗法能有效降低机体炎症指标及控制支气管-肺组织的感染。证明本研究方案适用于治疗痰热壅肺型支气管扩张急性加重期的患者,为中西医结合治疗支气管扩张急性加重期提供了新的治疗途径,为中医疗法应用于支气管扩张急性加重期提供一定的依据。更多还原

【Abstract】 ObjectiveThe aim of the study was to assess the effect of Qingjin Huatan decoction and Acupoint-injection Therapy on Acute Exacerbation Bronchiectasis, which affected clinical symptoms and sign, inflammation index. The result may offer guides in curing Acute Exacerbation Bronchiectasis with traditional Chinese medical science.Methods30Acute Exacerbation Bronchiectasis patients with retention of heat ang Phlegm and Heat syndrome in the lung were randomly assigned to treatment group (group T) or to control group (group C) on a single-blind basis. Group T accepted conventional therapy of the western medicine (controllability antibiotic, expectorants, hemostatic) plus Qingjin Huatan decoction and acupoint-injection, while group C purely accepted conventional therapy of the western medicine. The course was10days. The clinical symptoms and sign, inflammation index were observed and compared after10days.Results1.The management of Qingjin Huatan decoction and Acupoint-injection Therapy could improve the clinical symptoms and sign of Acute Exacerbation Bronchiectasis patients.The clinical symptoms and sign of both group have significant difference after ten-day treatment compared with those before the treatment (P<0.01). The clinical symptoms and sign of group T was better than group C with significant difference (P<0.05or P<0.01) after the treatment, which showed the management of group T is better.2. The management of Qingjin Huatan decoction and Acupoint-injection Therapy could remedy the state of inflammation and control the infection of bronchia-pulmonary.CRP and WBC of each group have significant difference (P<0.01) after the treatment compared with pre-treatment, that showed the management of both groups could reduce the CRP and WBC. After the treatment, CRP and WBC of group T was better than group C with significant difference (P<0.05). ConclusionThe management of the Qingjin Huatan decoction and Acupoint-injection Therapy can help Acute Exacerbation Bronchiectasis patients, it can improve the clinical symptoms and sign, remedy the state of inflammation and control the infection of bronchia-pulmonary. By meta analysis, the study offer a new method to cure Acute Exacerbation Bronchiectasis with traditional Chinese medical science combining with western medicine.更多还原
