山楂消脂胶囊对痰瘀证中低危高血压颈动脉血管重构干预

【摘要】 目的:本研究拟测定本院制剂山楂消脂胶囊对痰瘀互阻证中低危高血压患者颈动脉中膜厚度(IMT)、颈动脉血管内径(Dd),并观察血脂常规、血清内皮素(ET)、血管紧张素-Ⅱ(Ang-Ⅱ)治疗前后的变化,探讨山楂消脂胶囊对高血压血管重构干预的作用机制,从而为其扩大临床应用范围提供理论依据。方法:收集2010年10月至2011年10月在广州中医药大学附属佛山市中医院心血管内科住院治疗的痰瘀互结证中低危高血压患者60名,采用随机对照临床试验,分为对照组(常规治疗),治疗组(常规治疗+山楂消脂胶囊),两组治疗前均测定血脂常规、血清内皮素(ET)、血管紧张素-Ⅱ(Ang-Ⅱ)变化,B超测量颈动脉中膜厚度(IMT)、颈动脉血管内径(Dd)的前后变化;治疗前后监测:①血、尿、粪常规;②肝、肾功能;③血压变化。结果:治疗组在降低血压的显效性、降低血脂水平、降低血清内皮素(ET)、血管紧张素-Ⅱ(Ang-Ⅱ)浓度以及干预颈动脉中膜厚度(IMT)、颈动脉血管内径(Dd)等方面均优于对照组,且加用山楂消脂胶囊后无明显不良反应。结论:经实验室检查,治疗后治疗组血脂水平以及ET、Ang-Ⅱ较前降低,经检验,两组间及同组治疗前后差异具有统计学意义,故认为山楂消脂胶囊对颈动脉血管重构的干预作用,即减少IMT,增加Dd,其机制可能与降低血脂水平、降低血清内皮素(ET)及血管紧张素-Ⅱ(Ang-Ⅱ)血清浓度有关,为山楂消脂胶囊治疗高血压病提供了理论依据。

【Abstract】 Objet iveTo observe the concentrition change about blood lipids routine ET、Ang-II of serum and change of IMT、diameter of carotid artery about phlegm stagnation low risk patients of hypertension, and to find the mechanism of shanzha xiaozhi capsule, moreover to privde a theoretical basis for expanding range of clinical applications.MethodsA total60cases of phlegm stagnation low risk patients of hypertension during the October2010to February2012, randomly divided into two guoups,30in the control group were treated by conventional treatment, and30in the treating group were treated by onventional treatment and hawthorn xiaozhi capsule. Before and after the study, we observe the concentrition change about blood lipids routine、ET、Ang-II of serum and change of IMT、Dd of carotid artery; regular tests on blood、 urine、feces and blood pressure, and the function of the liver and kidneys.ResultsTreating group was better than control group about the effect of reduce the blood pressure、reduce blood lipids routine、ET、Ang-II of serum and change of IMT、Dd of carotid artery, and shanzha xiaozhi capsule have not adverse reactions.ConclusionBy the laboratory examination, the change of IMT、Dd of carotid artery have statistical significance on the same group and both groups. Maybe the mechanism of the change of IMT、Dd of carotid artery is reducing blood lipids routine、ET、Ang-Ⅱ of serum, this can provide a theoretical basis for shanzha xiaozhi capsule to cure hypertension.
