穴位按摩护理技术在中医外科和中医骨伤科的适用性筛选

【摘要】 研究目的对穴位按摩护理技术在中医外科和中医骨伤科进行操作安全性、护理专业性、护士胜任度和适用程度4个维度的筛选；通过层次分析法确定上述4个维度的权重,为穴位按摩护理技术在临床的开展和后期的相关研究提供参考。研究方法采用德尔菲(Delphi)专家函询法进行,运用文献分析法和半结构访谈法拟定筛选条目,从操作安全性,护理专业性,护士胜任度和适用程度4个维度进行筛选,经过预实验确定专家函询问卷,对26名临床医疗专家和临床护理专家进行了两轮函询；运用选择率、满分比、均数和变异系数对德尔菲(Delphi)专家函询结果进行统计,得出专家函询结果；根据第二轮函询结果运用层次分析法计算4个维度的权重；运用Excel2007、 SPSS17.0统计软件、层次分析法yaahp0.5.3软件进行数据处理。研究结果经过两轮专家函询,专家意见较为集中一致,筛选得到了10个临床应用性较好的条目,其中产后缺乳-胀痛,乳腺增生病-胀痛和脊柱骨折-皮肤受压(预防压疮)3个条目在4个维度上的均值排序居前,函询结果显示穴位按摩护理技术对解决上述三个临床护理问题有比较好的临床适用性。基于第二轮函询结果,使用层次分析法确定的权重结果显示,操作安全性(0.3403)>护理专业性(0.2891)>适用程度(0.1945)>护士胜任度(0.1761),说明操作安全性和护理专业性在评价穴位按摩护理技术的临床应用上较为重要。结论最终从23个条目四维度中筛选得到最适用的10个条目,为穴位按摩护理技术在中医外科和中医骨伤科的临床实际应用提供了参考依据；权重结果显示操作安全性和护理专业性是评价穴位按摩护理技术临床适用性的重要维度。
【Abstract】 ObjectiveScreening the point massage nursing operations suitable for Chinese medicine surgery and Chinese medicine Orthopedics and Traumatology in four dimensions, i.e., the safety of operation, specialty of nursing, competence of nurses and applicabilities.The weights are determined based on the four dimensions abovementioned via Analytic Hierarchy Process. The present study can provide references for the clinical of the point massage nursing operations and relevant researches in the future.MethodThe present study conducted via Delphi expert inquiry method following the four dimensions, i.e., the safety of operation, specialty of nursing, competence of nurses and applicabilities. Using the literature analysis and semi-structured interviews, the screening items can be determined. Twenty-six clinical doctors and clinical nursing experts were inquired for two rounds based on the preliminary expert inquiry Questionnaire. The expert inquiry results obtained based on the Delphi statistics using selection rate, full marks proportion, mean and coefficient of variation. The weights of the four dimensions were calculated based on the second expert inquiry results via Analytic Hierarchy Process. The data was analyzed using the commercial software, such as Excel2007, SPSS18.0and yaahp0.5.3.ResultsAfter two rounds expert inquiry, ten items which have better clinical applications were obtained via screening based on the consistent of the experts. Among them, the lack of lactation-gas pains, cyclomastopathy-gas pains and spinal fractures-skin compressed (prevention of press sores) come into the top three in the four dimensions. The inquiry results showed that the point massage nursing operations has good applicability in solving the abovementioned three clinical nursing issues.The weights are determined based on the two rounds inquiry results via Analytic Hierarchy showed that operation safety (0.3403)> nursing professional (0.2891)> suitable extent (0.1945)> nursing competent (0.1761).The results shows that the operation safety and nursing professional is vital in evaluation of the clinical application of the point massage nursing operations. ConclusionFinally, ten applicable items obtained from the twenty-three items via the screening can provide references for the clinical application of the point massage nursing operations in Chinese medicine surgery and Chinese medicine Orthopedics and Traumatology. The weights results show that operation safety and nursing professional are two important dimensions for evaluation of the clinical application of the point massage nursing operations.
