云南省曲靖市男男性行为者艾滋病高危行为调查研究

Investigation on the AIDS-related High Risk Behaviors among Men Who Have Sex with Men in Qujing of Yunnan Province

【摘要】 目的通过对云南省曲靖市男男性行为者艾滋病高危行为状况的调查研究,探讨该地区该人群艾滋病防制工作的重点。方法调查于2009年4月至10月进行,依托曲靖市疾病与预防控制中心下属的MSM外展服务机构--城南阳光工作室,招募10名MSM志愿者进行培训,经严格考核后4名合格成为调查员。采用方便抽样和“滚雪球”的方式接触目标人群,并采用国家级HIV综合监测点2009年MSM统一监测问卷对目标人群进行一对一问卷调查,督导员对调查员的工作进行全程督导检查,保证问卷质量。结果311名调查对象平均年龄为27.82±6.76岁。样本主要来源于酒吧/茶吧/夜总会,占43.1%。性取向以同性恋为主,占76.2%,没有异性恋。首次性行为年龄平均为19.78±3.66岁,最小14岁。首次性行为性伴为男性者占77.2%。首次男男性行为年龄平均为20.21±3.98岁,最小为14岁。近6个月发生过同性肛交性行为的占90.7%(282/311),同性性伴数平均3.05±2.68人,最多达17人,仅有24.1%(70/290)自述每次都使用安全套。最近一次同性肛交性行为安全套使用率为82.1%(238/290),且不同人群间存在差异。近6个月发生过异性性行为的男男性行为者占13.2%(41/311),异性性伴数平均1.05±0.22人,最多2人,仅有12.2%(5/41)自述每次都使用安全套。最近一次异性性行为安全套使用率为22.0%(9/41),且不同人群间存在差异。近6个月接受过同性商业性服务的29人(9.3%),接受的商业性伴数平均2.67±1.78人,最多7人,仅有24.1%(7/29)自述每次都使用安全套。最近一次接受同性商业性服务时安全套的使用率为82.8%(24/29)。近6个月提供过同性商业性服务的7人(2.3%),提供的商业性伴数平均3.50人,最多4人,仅有28.6%(2/7)自述每次都使用安全套,最近一次提供同性商业性服务时安全套的使用率为85.7%(6/7)。调查显示,宣传广告栏、电视和书籍是调查对象艾滋病知识和信息的主要来源。调查对象艾滋病知识总体知晓率为83.0%,UNGASS指标问题全部答对的199人,占64.0%。对艾滋病知识认知错误者,不同人群间存在差异。结论本调查结果表明,调查对象中多性伴和无保护肛交性行为较为常见;异性性行为时安全套使用率低;存在一定的同性商业性行为;对艾滋病知识的掌握存在薄弱环节。必需采取行之有效的干预措施,防止该人群感染和传播艾滋病。

【Abstract】 Objective To research AIDS related high-risk behavior among MSM (men who have sex with men) in Qujing city; and to determine the focus in the area of AIDS control in Qujing of Yunnan province.Method Survey in April 2009 to October, relying on the MSM outreach services -- South Sunshine Studios which under Qujing Disease Control and Prevention Center, recruited 4 MSM volunteers as an investigator, a "snowball" approach contacts target population, and using the MSM unified monitoring questionnaire of 2009 to face to face questionnaire survey. Supervisor supervised the work of investigators to ensure the quality of the questionnaire.Result 311 respondents an average age of 27.82±6.76 years. Samples mainly from the bar / Tea Bar / club, accounted for 43.1%. Sexual orientation as a homosexual, accounted for 76.2%, no heterosexual. Average age of first sex was 19.78±3.66 and the youngest 14 years old. First sex for male sexual partners accounted for 77.2%. Average age of the first sex with men was 20.21±3.98 years, minimum 14 years of age. Occurred in the past 6 months same-sex anal sex accounted for 90.7 %( 282/311), the average number of same-sex sexual partners was 3.05±2.68 and a maximum of 17, only 24.1% (70/290*) readme used condoms every time. Latest homosexual anal sex condom use rate was 82.1% (238/290*), and there are differences between different groups in using condom. Occurred in the past 6 months the heterosexual sex accounted for 13.2 %( 41/311), the average number of heterosexual partners was 1.05±0.22 and the maximum was 2, only 12.2% (5 / 41) readme used condoms every time. Recent heterosexual sex condom use rate was 22.0% (9 / 41), and the differences between different groups in using condom.In the past six months, the respondents of accepting commercial sex services was 9.3 %( 29/311), the average number of commercial sex partners was 2.67±1.78, and a maximum of 7. Only 24.1% (7 / 29) readme used condoms every time. Latest accepting same-sex commercial sex services condom use rate was 82.8% (24/29).In the past 6 months , those who provide same-sex commercial sex services was 2.3%(7/311), only 28.6% (2 / 7) readme use condoms every time, Latest providing same-sex commercial sex services condom use rate was 85.7% ( 6 / 7). Survey, publicity billboards, television and books were the main sources of AIDS knowledge and information. The awareness rate of AIDS knowledge was 83.0%. 64.0% of the MSM gave correct answers to all questions listed in UNGASS. And there are differences between different groups in erroneous understanting of AIDS transmission and prevention.Conclusion The results show that among the respondents multiple sexual partners and unprotected anal sex are more common. Heterosexual sex condom use rate is low. Same-sex commercial sex behaviors are not uncommon.MSM have largely insufficient knowledge of AIDS. Necessary to adopt effective interventions to prevent AIDS from infecting and spreading in this population.
