医院院长职业化管理的现状调查及对策研究

【摘要】 为进一步推进医院院长职业化管理的进程,本研究在充分查阅有关文献的基础上,采用了问卷调查、深度访谈等方法,定性与定量相结合,调查了政府主管卫生部门的管理人员、公立医院院长、医院的医护人员和社会公众,了解了医院院长职业化管理的整体现状。研究结果显示:广州市医院院长职业化管理的专业化和专职化程度不高,但院长们具有良好的管理愿望和素质条件,对职业化管理具有一定的认识,并肯定由专家型院长向职业化院长过渡的模式,医务人员和社会公众也接受和认可职业化院长来管理医院。医院院长职业化管理存在观念认识误区、素质能力与院长管理岗位存在一定差距、教育培训不能满足管理工作的需要等问题。影响广州地区公立医院院长职业化管理的因素有:现行的医院院长选拔任用制度与医院院长职业化管理的客观要求存在一定的矛盾、院长的绩效考核主体缺位和内容缺失、卫生管理职称系列的缺失、医院激励机制的建设与医院院长职业化管理的发展需要存在一定的差距、医院院长职业化管理的法律缺失等。针对上述问题,提出了如下对策和建议:培养院长职业化管理意识,营造医院院长职业化管理的氛围;优化院长的知识结构和能力结构,提高院长个人素质;建立医院院长职业化、任职资格准入、任职考核、多层次的激励、退任安置等保障制度;推动院长职业化管理法制化;建立健全卫生管理专业教育体系,落实卫生管理教育发展战略,规范职业化培训制度;推行职业化的医院管理模式,强化科学管理和创新。

【Abstract】 In this study, for further promote the director of the hospital professional management process, on the basis of he full access to the literature, use the methods of questionnaire、depth interviews, and so on, Combine the qualitative and quantitative, investigate the manager of the government health authorities, the director of the public hospital, the hospital medical staff and the general public, understand the overall actual state of the professional management about the director of the hospital.The results show that the professional management degree of specialization and day job about the director of the hospital in Guangzhou city is not high, but the director of the hospital have the good management desires and quality conditions, have a certain recognition on professional management, Confirm the director of the hospital transition from the expert type to the professional pattern, medical personnel and the public also accepted and approved the president of the hospital manage the hospital.The professional management has the following problems: the conception is misunderstanding, and there is a certain gap between the quality and ability of management positions with the president, education and training can not meet the needs of the management. The impact factors of the professional management of the public hospital director in Guangzhou city: the certain contradictions between the current selection of the director of the hospital and the requirement of the professional management; the absence of the performance appraisal subject and content; the absence of the health management professional series; the certain gap between the hospital encouraged mechanism and the development of professional management; the missing of law about the professional management on the director of the hospital.To address the above problems, I put forward to the following countermeasures and suggestions: train the awareness of the director professional management, create a professional management atmosphere on the director of the hospital; optimize the director’s knowledge structure and ability structure, improve the director’s personal qualities; establish the security systems on a professional, qualifications for access, assessment office, multi-level incentive, retirement settlement, and so on; promote the legalization of the professional management on the director of the hospital; establish a sound health management education system, put the health management education developing strategy into practice, regulate the professional training system; implement the professional hospital management model, strengthen the scientific management and innovation.
