DAPK1、RARβ及APC在甲状腺癌中的蛋白表达及意义

【摘要】 目的:探讨DAPK1、RARβ及APC三种抑癌基因,在甲状腺癌中的表达及其意义。方法:1、本课题选择3种已被确认的与腺癌相关的抑癌基因(DAPK1、RARβ和APC);2、采用免疫组织化学S-P法检测63例甲状腺癌、21例甲状腺腺瘤、13例桥本氏甲状腺炎、10例结节性甲状腺肿及12例正常甲状腺组织中DAPK1、RARβ及APC蛋白的表达。结果:1、DAPK1在甲状腺各种疾病组织中的蛋白表达差异有统计学意义(P<0.05),DAPK1蛋白表达的阳性率从高到低依次为正常组织(83.3%,10/12)>结节性甲状腺肿(60%,6/10)>桥本氏甲状腺炎(53.8%,7/13)>甲状腺腺瘤(47.6%,10/21)>甲状腺癌(28.6%,18/63);2、RARβ在甲状腺各种疾病组织中的蛋白表达差异有统计学意义(p<0.05),RARβ蛋白表达的阳性率从低到高依次为桥本式甲状腺炎(23.1%,3/13)<正常组织(33.3%,4/12)<结节性甲状腺肿(40.0%,4/10)<甲状腺腺瘤(61.9%,13/21)<甲状腺癌(76.2%,48/63);3、APC在甲状腺各种疾病组织中的蛋白表达差异无统计学意义(P>0.05);APC的蛋白表达的阳性率为正常组织(50.0%,6/12)、结节性甲状腺肿(30%,3/10)、桥本氏甲状腺炎(46.2%,6/13)、甲状腺腺瘤(38.1%,8/21)及甲状腺癌(44.4%,28/63);4、DAPK1和RARβ蛋白表达水平与PTC和FTC的临床分期、淋巴结转移密切相关;有淋巴结转移组的DAPK1阳性率为13.5%,明显低于无转移组(44.4%);有淋巴结转移组的RARβ阳性率为78.3%,明显高于无转移组(44.4%);5、DAPK1、RARP和APC的蛋白表达与年龄、性别和民族无关(P>0.05)。6、在甲状腺癌中,尚不能认为DAPK1与RARβ的蛋白表达之间有相关关系(P>0.05);尚不能认为RARβ与APC的蛋白表达之间有相关关系(p>0.05);DAPK1与APC的蛋白表达,在甲状腺癌中呈中度正相关(r=0.445,0.4<r<0.7,P=0.000,P<0.05)。结论:1、在甲状腺癌中,抑癌基因DAPK1的表达缺失与APC的表达有一定的相关性,DAPK1的表达与甲状腺癌的发生、淋巴结转移和预后密切相关,检测DAPK1可以作为预测甲状腺癌淋巴结转移潜能及预后的有价值的参考指标。2、RARβ蛋白在甲状腺癌中高表达,其表达与甲状腺癌的发生发展及淋巴结转移相关,检测RARβ可以作为预测甲状腺癌淋巴结转移潜能的有价值的参考指标。3、APC基因在甲状腺癌及正常组织中的蛋白表达差异不大,其作为甲状腺癌诊断标记物的价值有待进一步研究。

【Abstract】 Objective:To evaluate the expression and clinical significance of DAPK1、RARβand APC in thyroid carcinoma.Methods:1. In this study, three kinds of tumor suppressor genes and abnormal gland cancer methylation genes such as DAPK1、RARβand APC,are selected,which have already been confirmed.2. Immunohistochemical stain was used to determine the protein expression of these genes in 63 cases of thyroid cancers,21 cases of thyroid adenoma,13 cases of hashimoto’s thyroiditis,10 cases of nodular goiter and 12 cases of normal thyroid tissue. Results:1. The variance of DAPK1’s protein expression has statistical significance in the thyroid gland various pathological tissues (P<0.05), protein expression was followed by normal thyoid tissue (83.3%, 10/12)>nodular goiter (60%,6/10)>hashimoto’s thyroiditis (53.8%,7/13)>thyroid adenoma (47.6%,10/21)>thyroid cancer (28.6%,18/63); 2. The variance of RARβ’s protein expression has statistical significance in the thyroid gland various pathological tissues (P<0.05), protein expression was followed by hashimoto’s thyroiditis (23.1%,3 /13)<normal thyoid tissue (33.3%,4/12)<nodular goiter (40.0%,4/10)<thyroid adenoma (61.9%,13/21)<thyroid cancer (76.2%,48/63); 3. The variance of APC’s protein expression had not statistical significance in the thyroid gland various pathological tissues (P>0.05),and from total not significant change tendency; 4. There are statistical significance that DAPK1 and RARβis to clinical stage and lymphnode metastasis in thyroid cancer; 5. The protein expression of DAPK1、RARβand APC are nothing to do with age, gender and ethnic (P>0.05); 6.The expression of DAPK1 in thyroid cancer was positively correlated with the expression of APC (r=0.445,0.4<r<0.7, P=0.000, P <0.05).Conclusion:1. It suggests that the expression of DAPK1 is correlated with the carcinogenesis, progression, infiltration and lymph node metastasis of thyroid carcinoma, it can serves as important indicators for predicting lymph node metastasis in thyroid carcinoma.2. It suggests that the expression of RARβis correlated with the lymph node metastasis of thyroid carcinoma.3. The variance of APC’s protein expression has not significant change tendency, the value of becoming diagnosis mark of thyroid gland cancer need for further research.
