双J管结壳成分的临床研究

【摘要】 目的:用红外线光谱分析法确定和比较结石患者在无菌条件下放置的双J管晶体结壳与原发结石的组成成分。方法:对2010年11月-2011年4月间322例患者的尿石和52例双J管结壳进行红外线光谱分析,322例为外科手术取出的上尿路结石,52例为上述结石患者在膀胱镜(或输尿管镜)下拔除的双J管结壳。结石患者放置双J管的平均时间是57天。结果:结石患者的双J管结壳的成分有草酸钙(37例,其中13例草酸钙-磷灰石混合结石,3例草酸钙-无水尿酸结石),磷酸镁铵(3例),磷灰石(4例,其中3例为磷灰石-草酸钙混合结石),无水尿酸(8例)。结石患者原发结石成分有草酸钙(40例,其中14例草酸钙-磷灰石混合结石,4例草酸钙-无水尿酸结石),磷酸镁铵(2例),磷灰石(5例,其中4例为磷灰石-草酸钙混合结石),无水尿酸(5例)。结论:双J管短期放置时间不影响结壳的发生及成分。草酸钙是结石患者双J管结壳的主要成分类型。原发结石和双J管晶体结壳的主要成分相似。结石患者双J管结壳平均重量76.5mg。当不能得到原发结石标本时,双J管结壳的红外线光谱分析不失为一种能预测结石成分的可靠方法。

【Abstract】 Purpose:To determine the composition of crystalline encrustation on double J ureteral stent inserted under sterile conditions by infrared spectroscopy, in comparison with primary calculus in stone formers.Methods:322 primary calculuses and 52 double J ureteral stent encrustations between November 2010 and April 2011 were collected and analyzed with infrared spectroscopy,322 upper tract urinary calculi from stone formers were surgically removed,52 double J ureteral stent encrustations from stone formers were removed by cystoscopy(or ureteroscope). The mean dwelling time was 57 days for the stone formers.Results:The double J ureteral stent encrustations in stones formers are calcium oxalate(37, in 13 cases together with apatite and 3 cases together with uric acid), struvite(3), apatite(4,in 3 cases together with calcium oxalate), uric acid(8).It is also identified that the primary calculuses in stone formers are calcium oxalate(40, in 14 cases together with apatite and 4 cases together with uric acid), struvite(2), apatite(5, in 4 cases together with calcium oxalate), uric acid(5).Conclusions:The short duration of stenting time does not influence the presence and composition of encrustation.Calcium oxalate is the predominant type of encrustation on double J ureteral stent in stone formers.Both the composition of primary calculus in stone former and crystalline encrustation on double J ureteral stent are similar. The mean weight of double J ureteral encrustation of stone formers was 76.5mg. Infrared spectroscopy analysis of double J ureteral stent encrustations is a reliable method of predicting stone composition when the stone cannot be retrieved.
