医院微生物检测系统的设计与开发

【摘要】 本文介绍了《医院微生物检测系统的设计与开发》的关键技术。现代抗菌药物的出现是细菌感染防治手段的一大进展,尽管减少了感染的发病率和病死率。但抗菌药物在现代医学中的广泛应用对人体并不是无害的。尽管抗生素本身并不引起突变,也不诱导和创造耐药菌,然而近年来,随着人类对抗菌素大量、无序的广泛使用,导致耐药菌株大量出现和广泛传播,并呈逐年增加趋势,其耐药性常以多重耐药为特点.由此给医院感染带来极为不利的影响,快速、准确的细菌鉴定及耐药监测对有效控制医院感染、防治耐药菌株传播具有重要意义。医院感染管理中原来使用的软件系统是英国Sensititre(先德)微生物鉴定/药敏分析系统。原软件选择的是用MIC法进行药物敏感性试验,选择实验的药物都是厂‘家所提供的药敏板上的药物,药品固定,不能更改。致使实验受到一定的局限限制,即有些药物临床没有,而临床有的药物药敏板上没有。所以我们报告给临床提供用药参考时,范围就缩小了。本系统是对仪器法药物敏感试验的补充。能够手工输入所需要的抗菌素,并能根据CLSI最新标准调整所选药物分组及更改药物折点。从而按照CLSI最新标准针对不同细菌选择药物,尽量和医院目前用药一致。本文主要完成了微生物报告单录入、系统添加设置、库维护及安全设置几个功能模块的设计和实现。系统运行结果证明,本文所设计的医院微生物检测系统可以满足手工药敏报告发放。对医院的感染治疗针对性更强,对临床的指导用药更加直接,有助于减少滥用广谱抗生素从而选择出多重耐药的院内感染菌株。

【Abstract】 The key technologies for design and development of microorganism detection system in the hospital are presented in this paper. The presentation of modern antibacterial drug is a great process for bacteria infection prevention and treatment method, although it reduces the infectious disease incidence and fatality rate, the wide application of antibacterial drug in modern medical science does not do harm to body. Although antibiotics will not cause abrupt change itself, and not induce and create drug-resistant bacteria, however in recent years, the wide application of antibiotic largely and disorderly leads to the occurrence and wide spread of large quantities of drug-resistant strain, which show increase tendency year by year, the feature of drug-resistant is multiplex. Thereby, it brought the adverse effect on the infection in the hospital, fast and accurate bacteria identification and drug-resistant monitor is of an important significance on effectively controlling the infection in the hospital and preventing and treating the spread of drug-resistant strain. In infectious management in the hospital, the current software system is Sensititre microbiota inditification/drug sensitive analysis system made in England. MIC method is selected for drug sensitive test in the software, the selected drug is on drug sensitive plate, the drug is fixed and not be changed. In this way, some drugs do not exist in the clinic, but the drug in the clinic do not exist on the drug sensitive plate. Therefore, the range for the provided drug reported to the clinic is reduced. The system is the complement for instrument method drug senstive test, can input the needed antibiotic by mannual mode and adjust the selected adug group and change drug turning point according to the CLSI latest standard, select drug aimed at different bateria interm of the CLSI latest standard, thus keeping in line with the current drug in the hospital as possible.The design and implementation of function module, such as microbial report list input, add and setup of system, database maintenance and security setup, are completed in the paper. The result of system run showed that the system in the paper can meet the delivery of mannual drug sensitive report. The system has the stronger direction for infectious treatment in the hospital, more directly guide the drug application for the clinic and help to reduce the abuse of broad-spectrum antibiotics, thus selecting multiple drug-resistant infectious bacteria strain within the hospital.
