【摘要】 医院信息管理系统与医保系统的融合是目前的发展趋势,而两者之间的连接方式,是整合医疗保险业务系统成功运作的关键,采用医疗保险中介信息系统的运作模式是最佳选择。根据业务内容和使用对象的不同,医保中介系统提供了宏观决策、业务管理和医疗服务管理三方面的功能。利用医保中心资源数据库可以对医保基金的收支情况进行动态监控和分析预测,对政策执行情况进行评估,从而加快决策科学化进程,支持医保基金长期安全运行。本文对接口技术、中间件技术、医保数据管理、分布式组件封装技术、数据库设计和项目管理、SQL Server DBMS存储过程、医保政策参数管理的两个核心子系统的设计与实现进行了研究。主要内容有:详细分析、设计了医保业务管理系统、系统信息维护这两个核心子系统。采用灵活的系统配置,使得该系统可支持不同的医改模式和医改政策。在设计过程中采用面向对象的方法,容纳及封装医疗保险系统的约束机制,并通过业界公认的开放系统接口标准DCOM,以发行医疗保险行业规范的标准组件方式,把该约束机制嵌入医院信息管理系统的业务流程。研究中间件技术,尤其是基于对象请求代理的中间件技术,以此最终实现屏蔽不同的硬件平台、数据库、消息格式、通信协议之间的鸿沟与差异,提供HIS与医保系统之间的高效、便捷的通信能力。

【Abstract】 It is the trend for the Hospital Information Management System to participate in the operation of medical insurance system. The connection way between them is the key for the successful operation of the integrated medical insurance business system. The mode of medical insurance intermediary information system is the best choice.According to the business contents and differences of objects used, the medical insurance intermediary system mainly provides three kinds of functions, namely macro decision-making, business management and medical service management. The use of database of medical insurance center can carry out the work of dynamic monitoring, analyzing and predicting the income and expense information of medical insurance fund, and assessing the implementation of policies, which speeds up the process of scientific decision-making, as well makes the medical insurance fund run safely and lastingly.In this paper, the research is done in the following aspects:interface, middleware, medical insurance data management, distributed component packaging, database design and project management, storage process of SQL Server DBMS, the design and implementation of two core subsystem of medical insurance policy parameter management. The contents mainly include:Analyze in detail and design two core subsystems, namely the medical insurance business management system and information maintenance system. The system can support different medical reform modes and policies by adopting flexible system configuration.In the process of designing, adopt object-oriented method, contain and encapsulate the restraint mechanism of medical insurance system, and embed the mechanism into the business flow of the hospital information management system in the standard component mode through the open system interface-DCOM accepted by the trade people.Study the middleware technology, particularly, the technology based on object request agent, so as to shield different hardware platform, databases, message formats, gap and difference between communication protocols, as well offer highly effective and convenient communication capability between HIS and medical insurance system.
