【摘要】 目的：观察温阳化饮方对慢性充血性心力衰竭的临床疗效并探讨其作用机制。方法：根据纳入病例标准,选取60例慢性心力衰竭患者,随机分为治疗组和对照组。对照组30例,给予常规西药治疗；治疗组30例,在常规西药治疗基础上加服温阳化饮方,一日一剂,二次分服,4周后进行临床疗效评定。分别于治疗前、后行心电图及血、尿、便常规,电解质,肝肾功能等实验室检查,观察患者心功能分级(NYHA)、症状、体征及心脏彩超射血分数及每搏输出量、血浆氨基末端脑钠肽前体水平在治疗前后的变化,从而评判其疗效及安全性。结果：治疗4周后,临床研究显示治疗组在心功能、Lee氏心衰积分、中医证候总疗效方面优于对照组(P<0.05)；中医单项症状疗效分析表明：治疗组在畏寒肢冷、咳嗽、疲倦乏力、烦躁不安、腹胀、口唇青紫方面的改善情况明显优于对照组(P<0.05)；治疗后,治疗组心脏射血分数、每搏输出量改善优于对照组(P<0.05),血浆氨基末端脑钠肽前体水平降低幅度优于对照组(P<0.05)。结论：温阳化饮方对阳虚水泛证慢性充血性心力衰竭在改善临床症状、心功能、心脏彩超射血分数、每搏输出量及血浆氨基末端脑钠肽前体水平方面有显著疗效。更多还原

【Abstract】 Objective:To observe the clinical effect of Wen Yang HuaYin Fang in treating the chronic congestive heart failure (CHF) and explore its mechanism of its effect. Methods: According to the standards included in the case,60 cases of patients were divided randomly into two groups equally:treating group and control group. The control group with 30 cases were dosed with routine western medicine, and the treating group with 30 cases were dosed with Wen Yang HuaYin Fang on the basic treat of routine western medicine, and determined the curative effect after four weeks. Do the laboratory examination of ECG, blood routine, urinalysis, routine stool, electrolyte, liver and renal function before and after treatment, and observed The changes of cardiac function classification (NYHA), clinical symptoms or signs, ejection fraction (EF), stroke volume (SV) and plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) before and after treatment to determine the efficacy and security. Results:After 4 weeks’treatment, the clinical study showed that the treatment group was better than the control group in cardiac function, Lee’s failure points, total effect of Traditional Chinese Medicine (TCM) syndromes (P<0.05), so as the cold extremities, cough, fatigue, Irritability, abdominal distention and Lips cyanosis of symptoms of TCM (P<0.05), and the same with EF and SV (P<0.05), as well as the reduced degree of NT-proBNP (P<0.05). Conclusion:Wen Yang HuaYin Fang can ameliorate clinical symptoms, cardiac function, EF, SV and NT-proBNP in the treatment of syndrome of water overflowing due to yang deficiency of CHF.更多还原
