FRD宫颈上皮组织染色技术在阳春地区宫颈癌前病变筛查中的应用

【摘要】 目的:探讨叶酸受体介导的宫颈特殊染色法(FRD)宫颈上皮组织染色技术在阳春地区宫颈癌前病变筛查中的应用。方法:选取广东省阳春地区30~60岁的女性1 000例进行以人群为基础的宫颈癌筛查。均给予RFD检查和液基薄层细胞学(TCT)检查,并以阴道镜检查诊断结果作为金标准,比较RFD、TCT筛查宫颈癌前病变的效能,计算诊断符合度、敏感性、特异性。结果:阴道镜检查为宫颈正常或炎症875例、CINⅠ64例、CINⅡ36例、CINⅢ23例、宫颈癌2例,FRD检查阳性67例(6.70%)、阴性933例(93.30%),TCT检查阳性220例(22.00%)、阴性78例(78.00%),FRD宫颈癌前病变的诊断符合率高于TCT检查(x~2=95.230,P=0.000);FRD筛查宫颈癌前病变的敏感性、特异性均高于TCT检查,比较差异均有统计学意义(P<0.05)。结论:FRD宫颈上皮组织染色技术应用在阳春地区宫颈癌前病变中诊断符合率、敏感性和特异性极高,且是一种准确、快速、便捷、经济的一种检测方法,值得在临床上广泛推广。

【Abstract】 Objective:To explore the application of folate receptor mediated cervical special staining(FRD)in cervical precancerous lesion screening in Yangchun area.Method:A population-based screening for cervical cancer was conducted in 1 000 women aged 30 to 60 years old in Yangchun,Guangdong Province.RFD and liquid-based thin-layer cytology(TCT)were performed,and the diagnostic results of colposcopy were used as the gold standard.The efficacy of RFD and TCT in screening cervical precancerous lesions was compared,the diagnostic accuracy,sensitivity and specificity were calculated.Result:Colposcopic examination included 875 cases of cervical normal or inflammation,64 cases of CIN Ⅰ,36 cases of CIN Ⅱ,23 cases of CIN Ⅲ and 2 cases of cervical cancer.FRD was positive in 67 cases(6.70%)and negative in 933 cases(93.30%),TCT was positive in 220 cases(22.00%)and negative in 78 cases(78.00%),the diagnostic coincidence rate of FRD for cervical precancerous lesions was higher than that of TCT( x~2=95.230,P=0.000).The sensitivity and specificity of FRD in screening cervical precancerous lesions were higher than those of TCT,the differences were statistically significant(P<0.05).Conclusion:The application of FRD cervical epithelial tissue staining technology in Yangchun area has high diagnostic accuracy,sensitivity and specificity,and it is an accurate,fast,convenient and economical detection method.It is worth of popularizing widely in clinic.
