【摘要】 目的识别芦山地震后早期当地教师重型抑郁障碍(MDD)的时点患病率及高危因素。方法采用前瞻性队列研究,分别于地震后14~20d及85~95d对雅安市宝兴县教师进行地震后社会心理状况及MDD的诊断性评估。前者采用自编调查表,后者采用简明国际神经精神访谈(MINI)中MDD模块。采用单因素与多因素logistic回归分析MDD的危险因素。结果共319例教师完成了2次评估。地震后3月MDD的时点患病率为4.70%(15/319)。多因素logistic回归分析显示,MDD的高危因素有直系亲属受伤(OR=6.26)、住房不能居住(OR=4.26)、地震后14~20d内情绪表现为容易伤感(OR=11.45)。地震后14~20d内情绪反应表现为担心(OR=0.14)是MDD的保护性因素。结论地震后教师的心理状态值得重视,应重点关注有直系亲属受伤、住房不能居住、地震后14~20d内容易伤感或缺乏担心情绪反应的教师。更多还原

【Abstract】 Objective To determine the prevalence and risk factors of major depression disorder(MDD)in local teachers at an early stage post Lushan earthquake.Methods A prospective cohort study was undertaken,with 319 local teachers receiving diagnostic MDD assessment using MINI 14-22 dand 85-95 dafter the earthquake,respectively.Possible predictive factors were identified using a self-developed questionnaire. Univariate and multivariate logistic regression analyses were performed to determine the predictors of MDD.Results Fifteen(4.70%)local teachers were diagnosed with MDD.Those who had an injured direct relative 〔odds ratio(OR)=6.26)〕,with inhabitable house(OR=4.26),and felt easy to get sad 14-20 dweeks post earthquake(OR=11.45)were more likely to suffer from MDD than others.An anxious emotion 14-20 dpost earthquake was a protective factor(OR=0.14)of MDD.Conclusion Post earthquake MDD in local teachers deserves concerns,especially for those who have injured relatives,with an inhabitable house,and who feel easy to get sad 14-20 dpost earthquake.更多还原
