桑仙颗粒改善老年高血压病患者脉压和动脉弹性功能的临床与实验研究

【摘要】 目的:评价以补肾活血为功效的桑仙颗粒对老年高血压病患者脉压和动脉弹性功能的影响并探讨作用机制。方法:临床研究选择肾虚血瘀型老年高血压病患者86例,随机分为试验组与对照组,对照组口服吲哒帕胺,试验组在此基础上给予桑仙颗粒治疗,疗程12周。实验研究选择10周龄SHR大鼠,分组给予桑仙颗粒和吲哒帕胺灌胃治疗,观察桑仙颗粒的降压效应和可能的作用机理。结果:临床实验显示,桑仙颗粒可降低血压和脉压水平,调节血脂,改善临床症状和动脉僵硬度指数、脉搏波传导速度等动脉弹性功能,统计学处理表明本药大部分疗效优于对照组。动物实验中,桑仙颗粒能明显控制血压、调节血管活性物质ET、NO、TXB2、6-keto-PGF的释放,下调主动脉TGF-β1、NF-kB受体表达,改善血管平滑肌细胞超微结构,保护主动脉内皮。结论:桑仙颗粒有效降低血压、缩小脉压水平、改善动脉弹性功能,优于单纯降压治疗,其机制与多靶点调节内皮功能有关。

【Abstract】 Objective: Appraise an impact of the Sang Xian granule(SXG) which taking nourishing kidney and promoting blood circulation as effect over the elderly hypertension’ s pulse pressure and artery elastic function and discuss the effect mechanism. Methods: 86 cases of elderly hypertension belong to the sydrome of deficiency of kidney and stagnation of blood were observed in clinical trial.All cases were randomly divided into test group and control group, The control group oral Indapamide.and on the basis of this treatment,the test group were treated by SXG, the treatment lasted for 12 weeks.In experimental studies, 10-week-old SHR were given SXG or indapamide respectively by gavage treatment to observe antihypertensive effects and possible mechanism of SXG. Results: The clinical research showed that SXG can lower blood pressure and pulse pressure levels, regulate blood lipids, improve clinical symptoms and arterial elasticity function. Statistics showed that SXG is effective than the control group mostly. In experimental studies, SXG can control blood pressure obviously, regulate vasoactive substances such as ET、NO、TXB2 and 6-keto-PGF, inhibit the expression of TGF-β1 and NF-kB in aorta, improve ultrastructure of vascular smooth muscle cells(VSMC) and protect endothelial cells. Conclusion: SXG can lower blood pressure, reduce pulse pressure levels and improve artery elastic function effectively. The effect is better than antihypertensive treatment simply, the mechanism is related to the function of protecting endothelial function.
