正常股骨中下段骨髓MRI和~1HMRS研究

Study of MRI and ~1HMRS on Normal Middle and Distal Femoral Bone Marrow

【摘要】 目的1、采取定性和定量的方法研究2~30岁正常股骨中下段骨髓磁共振(MRI)信号强度随年龄增长的变化特点,为骨髓基础研究和骨髓疾病分析提供参考依据。2、采取实验性临床研究来选择符合股骨骨髓腔解剖学实际情况、检查时间短、重复性良好、图像质量高而且临床实用性强的磁共振波谱(1HMRS)采集方法。3、分析不同年龄阶段正常股骨中下段骨髓1HMRS在骨髓生理转化过程中的波形特征,从分子水平揭示骨髓组织生理学转化规律,同时为临床研究骨髓疾病代谢提供正常参考依据。材料与方法采用德国西门子公司Magneton Vision 1.5 T全身超导磁共振扫描仪的标准正交头部线圈和随机配备的单体素氢波谱软件对受检者右侧股骨中下段骨髓影像学进行前瞻性的研究,所有检查均在受检者本人或其家属的知情同意下完成。首先对13名正常在校生志愿者股骨中下段骨髓进行~1HMRS的探索性研究,年龄16~33岁,男10名、女3名。其中7名股骨骨髓~1HMRS采用128次采集、1.0×1.0×1.0 cm~3大小的感兴趣容积(VOI)而且同一部位重复采集2~3个VOI的方法,另外6名股骨骨髓1HMRS分别采用128和256次采集、1.0×1.0×1.0 cm~3和2×2×2 cm~3大小VOI的方法。继而随机选择2004年11月至2005年10月期间的72例2~30岁的正常股骨受检中国人,男51例,女21例,受检者为正常志愿者和无骨髓相关疾病者(未有化疗、放疗或激素等免疫抑制剂治疗史)。根据受检者年龄不同,参照国外Moore的年龄组划分方法将72例影像学资料分为6组:A组(2~5岁)9例,B组(6~10岁)10例,C组(11~15岁)10例,D组(16~20岁)13例,E组(21~25岁)14例,F组(26~30岁)16

【Abstract】 Objectives 1. To discuss the age-related changes of signal intensity on magnetic resonance imaging (MRI) in normal middle and distal femoral bone marrow with qualitatively and quantitatively to supply normal reference for basic research and clinical diseases of bone marrow. 2. To choose proper proton magnetic resonance spectroscopic (1HMRS) parameters to fit for practical femoral marrow cavity and to produce short-timed, well producible and excellent 1HMRS. 3. To investigate spectroscopic characteristics of 1HMRS of normal middle and distal femoral bone marrow within different ages, and to reveal histological bone marrow conversion and supply normal reference for clinical bone marrow diseases.Materials and methods The prospective study of right middle and distal femoral bone marrow was performed with a Siemens Magneton Vision 1.5-T system and head coil accompanied with a single-voxel proton MR spectroscopy software. All examinations were finished after the informed consent.Firstly, thirteen normal volunteers aged 16 to 33 years in our school prospectively enrolled in the tentative study of ~1HMRS on normal femoral bone marrow. ~1HMRS parameters for 7 volunteers were TR 1500 ms/ TE 270 ms, number of acquisition was 128, and volume of interest (VOI) size was 1×1×1 cm~3 and repeated 2~3 times within the same location. 1HMRS parameters for another 6 volunteers were TR 1500 ms/TE 270 ms and different numbers of acquisition (128 and 256, respectively) and different VOI sizes ( 2×2×2 cm~3 and 1.0×1.0×1.0 cm~3 , respectively).Secondly, seventy-two cases with normal femoral bone marrow were collected between Nov. 2004 and Oct. 2005.
