幽胃康治疗幽门螺杆菌相关性胃炎的临床与实验研究

【摘要】 目的：探讨幽胃康对幽门螺杆菌相关性胃炎（HPCG）的治疗作用及疗愈机理。方法：临床选择HPCG患者97例，随机分为两组，试验组（50例）给予幽胃康，对照组（47例）给予胃复春，观察两组患者临床及病理疗效。实验研究观察幽胃康对实验性慢性胃炎大鼠血清IL—8、胃粘膜PGE₂、cAMP、cGMP、SOD、MDA含量的影响与病理组织学改变，以及对小鼠胃排空运动的作用。结果：临床研究显示，试验组的临床总有效率（94％）、治愈率（36％）明显高于对照组（85.11％，21.28％），差异显著（P＜0.05）；改善慢性炎症和消退急性活动性炎症的疗效及H.pylori根除率皆明显优于胃复春（P＜0.05）；对胃粘膜萎缩、肠化及异型增生也显示出一定的缓解和逆转作用。实验研究表明，幽胃康能明显降低实验性慢性胃炎大鼠血清IL-8的含量，增加该大鼠胃粘膜PGE₂、cAMP及SOD的含量，提高胃粘膜cAMP/cGMP值，降低胃粘膜MDA含量，促进小鼠胃排空。组织形态学观察，试验组炎症消退理想，粘膜固有腺体萎缩改善明显。结论：幽胃康是治疗HPCG的有效方药，其机理可能与抗炎、抗氧化、保护胃粘膜及促进胃排空有关。更多还原

【Abstract】 Objective: To observe the therapeuc effect and explore the mechanism of Youweikang (YWK) in treating Chronic Gastritis with I-Ielicobacter Py- ion Infection (HPCG). Methods: 97 cases of HPCG were divided into two groups randomly. The treatment group (50 cases) was treated with YWK o- rally and the coiiIrOl group (47 cases) was treated with Weifuchun (WFC) orally. The clinical and pathological effects were observed during the course. The expcriment was designed to observe YWK influence on chronic gastritis rats model in the content of serum IL ?8, gastric mucosa PGE2, cAMP, cGMP, SOD, MDA and histopathology. Results: The clinical study has showed that the best rate and total rate in treatment group were 36% and 94% ,while in control group were 21. 28% and 85. 11% respectively. The difference was significant (P < 0. 05). The clinical study has also showed that YWK could improve the inflammation of gastric mucosa, clear Helicobacter Pylon (H. pylon ) and reverse atrophic gastric mucosa, in- testinal met aplasia and atypical hyperplasia. The animal experiment study showed that YWK could reduce the content of IL-8 and MDA significantly, increase the content of PGE2, cAMP and SOD, increasing cAMP/cGMP val- ue of gastric mucosa, improve gastric emptying. Conclusiom: YWK was an effective drug in treating J-IPCG. The mechanism might be realized by anti- inflammation, anti-toxidation, protect gastric mucosa and improve gastric emptying更多还原
