【摘要】 目的：探讨湖南省五市九家三级医院骨盆骨折的流行病学特点及救治现状,为政府相关决策的制定提供初步研究资料,以提高骨盆骨折的防治水平和救治质量,减少死亡率和伤残率。方法：收集2012年10月～2013年3月湖南省五市九家三级医院收治的65例骨盆骨折患者的临床资料,统计分析患者流行病学相关信息,院前急救及院内救治等情况。运用统计学软件SPSS13.0进行统计学分析。结果：65例患者中,男44例,女21例,男女比例为2.10：1.00。年龄为11-86岁,20～59岁占74.6%。交通伤30例(46.2%),高处坠落伤22例(33.8%)。73.8%的患者合并其它部位损伤。平均输血量Tile C型(1905.56m1)大于Tile A型(483.33m1),有统计学差异(P<0.05),大于Tile B型(1026.92m1),有统计学差异(P<0.05),Tile A型与Tile B型无统计学差异。出院时预后情况Tile C型比Tile A型更差,有统计学意义(P<0.05)。17例手术患者全部选择了内固定治疗。创伤重度评分(Injury Severity Score, ISS)与患者的总费用正相关,与患者的预后负相关。结论：湖南省五市九家三级医院骨盆骨折患者以中青年男性为主；高能量损伤是导致骨盆骨折的主要原因；Tile C型骨盆骨折输血量大,预后差;ISS评分能较好的反应患者的伤情和预测患者的预后。更多还原

【Abstract】 Objectives:To analyze the epidemiological characteristics of pelvic fractures in five city of Hunan, so that prevention and treatment of the injury can benefit from the study.Methods:The clinical data of the65patients with pelvic fractures, which admitted at9third-grade hospitals in Hunan, from October2012to March2013were collected. Their epidemiological information, pre-hospital and in-hospital situation were analyzed with SPSS13.0.Results:There are44male and21female patients, the male-to-female ratio is about2.10:1.00. The age range of patients was11to86years.74.6%patients was at age20to59.30patient(46.2%) was traffic injury,22patient(33.8%) was fall injury.73.8%patient suffered associated injuries which involved all over the body. By Tile’s classification of pelvic fractures, compare with type A and type B, type C needs more blood transfusion and its prognosis is worse. All surgical patients chose internal fixation. There is a very strong positive correlation between ISS and the cost. ISS is in negative correlation to the prognosis Conclusions:Pelvic fractures are mostly caused by high-energy injuries, often in young and middle-aged males.
