头风汤治疗偏头痛的临床疗效观察

【摘要】 目的 研究头风汤治疗偏头痛的临床疗效，同时初步探讨头风汤组成方义及其主药在治疗偏头痛中的作用机理。 方法 将所纳入的72例患者随机分为两组：治疗组36例，给予头风汤1剂／日，分两次服用；对照组36例，给予尼莫地平20mg，3次／日。均连续3周为一疗程。分别观察两组患者症状、体征以及TCD检查、血液流变学指标变化。 结果 1．治疗组总有效率为91.67％(痊愈25％，显效44.4％)；对照组总有效率为50％(痊愈8.3％，显效16.7％)。两组总有效率有显著性差异，P＜0.01。2．治疗2周时，治疗组和对照组头痛发作次数分别为(1.68±2.88)和(2.96±3.04)次，P＜0.01；持续时间分别为(10.27±18.72)和(20.06±28.05)h，P＜0.01。治疗3周时，两组头痛发作次数分别为(0.93±2.26)和(2.41±3.10)次，P＜0.01；持续时间分别为(5.64±16.53)和(18.96±30.15)h，P＜0.01．3．治疗前后中医证候量化计分减少分数，其差值治疗组为(9.29±1.12)、对照组为(4.65±1.98)，治疗组明显优于对照组，P＜0.01。4．两组患者治疗前后TCD和血液流变学指标的改善，治疗组明显优于对照组，P＜0.01。 结论 1．头风汤治疗偏头痛获较好疗效，与对照组有非常显著性差异。2．头风汤可搜风通络、祛瘀止痛，且可降逆除痰；可缓解大脑血管平滑肌痉挛、改善血液流变学异常，降低血小板聚集，减少5—HT产生，清除氧自由基，从而增加脑血流、改善脑血循、恢复脑血管舒缩功能及大脑正常供血供氧、降低颅内5—HT等物质含量、补充Mg+，达到解痉、镇静、止痛的作用。更多还原

【Abstract】 ObjectiveTo observe the effect of ToufengTang in patients and explore the theory and mechanism of TCM on treating the Migraine. MethodsSeventy-two cases were randomly divided into two groups: Thirty-six patients as treatment group who received ToufengTang ,once a day ,twice drinking ;and thirty-six patients as control group were treated Nimodipine Tablets 20mg, third one day too. Continuous three weeks as one course, the symptoms, signs, TCD and haemorheology parameters were observed before and after therapy. ResultsSeventy-two patients were treated for three months. First when therapy was finished, the total effective rate of treatment group was 91.67%(Cure rate: 25%, significantly effective rate: 44.4%) and the total effective rate of control group was 50% (Cure rate: 8.3%, significantly effective rate: 16.7%).The group’s total effective was significantly different, P<0.01.second.When the therapy finished by two weeks. Headache rate of treatment group and control group were (1.68 ?2.88)and (2.96 ?3.04),P<0.01; Lasting for time of headache were (10.27 ?18.72)and (20.06 ?28.05) hours , P<0.01. By three weeks , headache rate of the groups were (0.93 ?2.26)and (2.41 ?3.10), P<0.01; Lasting for time of headache were (5.64 ?16.53)and (18.96 ?30.15)hours ,P<0.01 . Third ,with the analysis of statistics , the TCM symptoms decreased scores measure and counting in treatment group were superior to the control one before and after therapy , PO.01. Correcting the TCD and blood rheology before and after therapy , there was obvious difference between the treatment group and control one , PO.01. ConclutionThese results suggested the ToufengTang in treating the Migraine have obvious clinical effective , these was a difference between the treatment group and control one .It was put forword that its mechanism was to relax the hyperkinesias of Vascular Smooth Muscle ,and to correct blood rheology abnormality , reduce BPC collecting , cut down 5-HT ,delete oxygen free gene ,so that it can promote the supply of blood and oxygen of the brain ,add the Mg-f, have effectives of spasmolysis and analgesia and relieving pain.更多还原
