不孕症妇女阴道微生态检测结果分析

【摘要】 目的 分析不孕症妇女阴道微生态检测结果。方法 选取2020年8月—2021年7月广东省生殖医院收治的159例不孕症妇女为研究对象,分别应用生理盐水涂片镜检、形态学检测、功能学检测、形态学与功能学结合法检测阴道分泌物,比较上述4种方法检测阴道分泌物的阳性率和一致性。不孕症妇女生理盐水涂片镜检阴道分泌物清洁度正常者,在阴道超声监测排卵、指导同房14 d后,应用全自动化学发光免疫分析仪检测血清β-人绒毛膜促性腺激素(β-hCG)水平,根据血清β-hCG水平分为未妊娠组和妊娠组,比较妊娠组和未妊娠组的阴道微生态检测结果。结果 159例不孕症妇女中,阴道分泌物生理盐水涂片镜检清洁度异常23例,阳性率14.5%;形态学检测结果异常111例,阳性率69.8%;功能学检测结果异常116例,阳性率73.0%;形态学与功能学结合法检测结果异常130例,阳性率81.8%。不孕症妇女阴道分泌物形态学检测、功能学检测、形态学与功能学结合法检测的阳性率均显著高于生理盐水涂片镜检阳性率(均P<0.05);不孕症妇女阴道分泌物形态学与功能学结合法检测的阳性率显著高于形态学检测阳性率(P<0.05)。不孕症妇女阴道分泌物形态学检测、功能学检测阳性率比较差异无统计学意义(χ~2=0.385,P>0.05)。未妊娠组微生态失调率、优势菌为非革兰阳性杆菌比例及过氧化氢异常率均显著高于妊娠组(均P<0.05)。结论 形态学与功能学结合法检测不孕症妇女阴道微生态简便易行,能为不孕症妇女的诊治提供有价值的信息。

【Abstract】 Objective To analyze the detection results of vaginal microecosystem in infertile women. Methods From August 2020 to July 2021, 159 infertile women treated in Guangdong Reproductive Hospital were selected, normal saline smear microscopy, morphological detection, functional detection, and morphological detection combined with functional detection were used to detect vaginal secretion, the positive rates and consistency of four methods were compared. Among the infertile women, the cases with normal cleanliness of vaginal secretion of normal saline smear microscopy underwent ovulation monitoring of transvaginal ultrasonography and resumed sex life for 14 days, then automatic chemiluminescence immunoanalyzer was used to detect serum β-human chorionic gonadotropin(β-hCG), according to the levels of serum β-hCG, the cases were divided into non-pregnancy group and pregnancy group, the detection results of vaginal microecosystem in non-pregnancy group and pregnancy group were compared. Results Among 159 infertile women, 23 cases were found with abnormal cleanliness of vaginal secretion of normal saline smear microscopy, the positive rate was 14.5%; 11 cases were found with abnormal result of morphological detection, the positive rate was 69.8%; 116 cases were found with abnormal result of functional detection, the positive rate was 73.0%; 130 cases were found with abnormal result of morphological detection combined with functional detection, the positive rate was 81.8%. The positive rates of morphological detection, functional detection, morphological detection combined with functional detection were statistically significantly higher than those of normal saline smear microscopy(P<0.05); the positive rate of morphological detection combined with functional detection was statistically significantly higher than that of morphological detection(P<0.05);there was no statistically significant difference in positive rate between morphological detection and functional detection(χ~2=0.385, P>0.05). The incidence rate of microecosystem disorder, the proportion of non-Gram-positive bacilli as dominant bacteria, and abnormal rate of hydrogen peroxide in non-pregnancy group were statistically significantly higher than those in pregnancy group(P<0.05). Conclusion Morphological detection combined with functional detection is simple and easy, which can provide valuable data for diagnosis and treatment of infertile women.
