不同体格测量指标与2型糖尿病发病风险关系的病例对照研究
Relationship between different anthropometric parameters and risk of type 2 diabetes mellitus: a case-control study
【摘要】 目的 探讨不同体格测量指标与2型糖尿病发病风险之间的关系,为早期预防2型糖尿病提供科学依据。方法 纳入299对2型糖尿病患者和健康对照。运用Logistic回归模型分析BMI、腰围、腰臀比和腰围身高比与2型糖尿病发病风险之间的关系。结果 在校正可能的混杂因素后,与正常组(BMI 18.5~23.9 kg/m~2)相比,超重和肥胖组(BMI 24.0~27.9 kg/m~2和≥28.0 kg/m~2)发生2型糖尿病风险的OR (95%CI)值分别是2.35 (1.56~3.57)和6.47(2.92~14.33)。相比于正常腰围组(男<85 cm,女<80 cm),中心性肥胖前期组(男85~89.9 cm,女80~84.9 cm)和中心性肥胖组(男≥90 cm,女≥85 cm)发生2型糖尿病风险的OR值(95%CI)分别是2.99(1.81~4.95)和3.88(2.43~6.22)。腰臀比最高组(Q4)和腰围身高比最高组(Q4)发生2型糖尿病的风险是最低组(Q1)的4.35倍与3.45倍(95%CI:2.48~7.64,1.94~6.14,P <0.001)。结论 超重、全身性肥胖、中心性肥胖前期和中心性肥胖可能增加2型糖尿病的发病风险。
【Abstract】 Objective To investigate the association between different anthropometric parameters and the risk of type 2 diabetes mellitus(T2DM), aiming to provide scientific evidence for early prevention of T2DM. Methods 299 pairs of T2DM patients and healthy controls were recruited in this case-control study. Logistic regression models were used to analyze the association between BMI, waist circumference(WC), waist-hip ratio(WHR) and waist-to-height ratio(WHtR) and the risk of T2DM. Results After potential confounding factors were adjusted, compared with the healthy BMI group(BMI 18.5-23.9 kg/m~2), the odds ratio(OR, 95%CI) in the overweight(BMI 24.0-27.9 kg/m~2) and obesity groups(BMI≥28.0 kg/m~2) was 2.35(1.56-3.57) and 6.47(2.92-14.33), respectively. Compared with the normal WC(male < 85 cm, female < 80 cm), the OR(95%CI) of T2DM risk in the pre-central(male 85-89.9 cm, female 80-84.9 cm) and central obesity(male ≥ 90 cm, female ≥ 85 cm) groups was 2.99(1.81, 4.95) and 3.88(2.43, 6.22), respectively. The risk of T2DM was 4.35 times(95%CI: 2.48-7.64, P < 0.001) higher in the highest WHR group(Q4) and 3.45times(95%CI: 1.94-6.14, P < 0.001) higher in the highest WHtR group(Q4) compared with those in the lowest WHR and WHtR groups(Q1). Conclusion Overweight, general obesity, pre-central and central obesity may increase the risk of T2DM.
