【摘要】 目的 探究鼻内镜下鼻中隔矫正术加鼻窦开放治疗慢性鼻窦炎伴鼻中隔偏曲（deviation of nasal septum,DNS）的效果。方法 回顾性选取2022年1月—2024年1月宁德市中医院收治的78例慢性鼻窦炎伴DNS患者的临床资料，按治疗方案不同将患者分为对照组和观察组，各39例。对照组予以鼻内镜下鼻窦开放手术，观察组予以鼻内镜下同期行鼻中隔矫正术加鼻窦开放手术。比较两组临床疗效、并发症发生率、症状评分、鼻腔鼻窦情况。结果 观察组治疗总有效率为94.87%(37/39），高于对照组的79.49%(31/39），差异有统计学意义（χ~2=4.129,P<0.05）。观察组并发症总发生率低于对照组，差异有统计学意义（P<0.05）。术后3个月，观察组鼻塞、脓涕、嗅觉减退、呼吸困难症状评分均低于对照组，差异有统计学意义（P均<0.05）。术后3个月，观察组鼻内镜CT Lund-Mackay评分、鼻腔鼻窦结局测试-22量表评分均低于对照组，差异有统计学意义（P均<0.05）。结论 鼻内镜下鼻中隔矫正术加鼻窦开放治疗慢性鼻窦炎伴DNS的疗效确切，同时也能明显减少并发症发生率，促进症状消退，加快鼻腔鼻窦功能恢复。更多还原

【Abstract】 Objective To explore the effect of endoscopic nasal septum correction combined with sinus opening in the treatment of chronic sinusitis with deviation of nasal septum (DNS).Methods The clinical data of 78 patients with chronic sinusitis and DNS admitted to Ningde Traditional Chinese Medicine Hospital from January 2022 to January2024 were retrospectively selected.The patients were divided into control group and observation group according to different treatment schemes,with 39 cases in each group.The control group was treated with open sinus surgery under nasal endoscopy,and the observation group was treated with nasal septum correction and open sinus surgery under nasal endoscopy at the same time.The clinical efficacy,complication rate,symptom scores,nasal cavity and paranasal sinuses were compared between the two groups.Results The total effective rate of the observation group was 94.87%(37/39),which was higher than 79.49%(31/39) of the control group,and the difference was statistically significant(χ~2=4.129,P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant (P<0.05).At 3 months after operation,the symptom scores of nasal obstruction,purulent discharge,hyposmia and dyspnea in the observation group were lower than those in the control group,and the differences were statistically significant (all P<0.05).At 3 months after operation,the nasal endoscopic CT Lund-Mackay scores and 22-item Sinonasal Outcome Test scores of the observation group were lower than those of the control group,and the differences were statistically significant (both P<0.05).Conclusion Endoscopic nasal septum correction combined with sinus opening is effective in the treatment of chronic sinusitis with DNS,which can significantly reduce the incidence of complications,promote the disappearance of symptoms,and accelerate the recovery of nasal cavity and sinus function.更多还原
