广泛性焦虑障碍患者氧化应激因子与焦虑症状的相关性分析

【摘要】 目的 探讨广泛性焦虑障碍(GAD)患者氧化应激因子与焦虑症状的相关性。方法 采用病例对照的研究方法,选取2022年1月至2023年1月在湖州市第三人民医院门诊就诊的GAD患者101例和健康体检者100名(对照组)。采用酶联免疫吸附试验测定静脉血氧化应激因子水平,包括丙二醛(MDA)、超氧化物歧化酶(SOD)、HDL、谷胱甘肽过氧化物酶(GSH-Px)、过氧化氢酶(CAT)、皮质醇、一氧化氮(NO)和脂质过氧化物(LPO),采用汉密尔顿焦虑量表(HAMA)评估焦虑程度,汉密尔顿抑郁量表(HAMD-17)评估抑郁程度。结果 GAD组MDA、皮质醇、LPO水平、HAMA评分和HAMD-17评分均明显高于对照组(均P<0.05);SOD、GSH-Px、CAT水平明显低于对照组(均P<0.05);两组间HDL、NO水平比较差异均无统计学意义(均P>0.05)。GAD组MDA、NO、LPO与HAMA评分均呈正相关(均P<0.01);SOD、GSH-Px与HAMA评分均呈负相关(均P<0.01)。经二元logistic回归分析显示,在控制性别、年龄、BMI和教育年限等因素后,MDA是GAD发生的危险因素(P<0.05)。结论 GAD患者存在氧化应激失衡,MDA水平升高是GAD发病的危险因素。

【Abstract】 Objective To explore the association of serum levels of oxidative stress factors with anxiety symptoms in patients with generalized anxiety disorder(GAD). Methods One hundred and one patients with GAD in Huzhou Third Municipal People’s Hospital from January 2022 to January 2023(GAD group) and 100 healthy subjects(control group) were enrolled in the study. The serum levels of malondialdehyde(MDA), superoxide dismutase(SOD),high density lipoprotein(HDL), glutathione peroxidase(GSH-Px), catalase(CAT), cortisol, nitric oxide(NO), and lipid peroxide(LPO), oxidative stress factors were determined by ELISA. Hamilton anxiety scale(HAMA) scores were measured to evaluate the degree of anxiety; and Hamilton depression scale-17(HAMD-17) scores were measured to evaluate the degree of depression in all subjects. Results The levels of MDA, cortisol, LPO, HAMA score and HAMD-17 score in GAD group were significantly higher than those in control group(all P < 0.05); while SOD, GSH-Px and CAT levels in GAD group were significantly lower than those in control group(all P < 0.05). There were no-significant differences in HDL and NO between the two groups(both P > 0.05). The levels of MDA, NO and LPO in GAD group were positively correlated with HAMA scores(all P < 0.05), while SOD and GSH-Px were negatively correlated with HAMA scores(both P < 0.05) in GAD patients. The regression model revealed that elevated MDA level is a risk factor for the onset of GAD(P < 0.05). Conclusion GAD patients have oxidative stress imbalance, and elevated MDA is a risk factor for the onset of GAD.
