增强超声造影联合BI-RADS分级区分乳腺良恶性病变的价值分析
The value of contrast-enhanced ultrasonund combined with BI-RADS grading in differentiating benign and malignant breast lesions
【摘要】 目的 分析增强超声造影联合乳腺影像报告和数据系统(BI-RADS)分级区分乳腺良恶性病变的诊断价值。方法 回顾2021年1月至2022年1月温州医科大学附属第一医院就诊后发现乳腺肿块100例患者的临床资料。所有患者均进行增强超声造影检查,并应用BI-RADS分级诊断,以病理检查结果为金标准,比较增强超声造影检查和BI-RADS分级单独及联合检测对乳腺良恶性病变的诊断价值。结果 根据病理检查结果将100例患者分为恶性病变组37例和良性病变组63例。与良性病变组比较,恶性病变组的增强开始时间、达峰时间均明显缩短,峰值强度明显增大(均P<0.05)。与单独检查比较,增强超声造影检查联合BI-RADS分级的特异度、准确度、阳性预测值均明显升高(均P<0.05),而灵敏度与阴性预测值比较差异均无统计学意义(均P>0.05)。结论 对乳腺良恶性病变进行鉴别诊断时可联合应用增强超声造影和BI-RADS分级,两者联合检测具有较高的诊断价值,可提高诊断准确率。
【Abstract】 Objective To investigate the value of contrast-enhanced ultrasonund(CEUS) combined with breast imaging reporting and data system(BI-RADS) grading in distinguishing benign and malignant breast lesions. Methods The clinical data of 100 patients with breast masses admitted in the First Affiliated Hospital of Wenzhou Medical University from January 2021 to January 2022 were retrospectively reviewed. Using postoperative pathological examination as the gold standard, the diagnostic value of CEUS and/or BI-RADS grading for distinguishing benign and malignant breast lesions was analyzed. Results According to the results of pathological examination, there were 37 cases of malignant lesions and 63 cases of benign lesions. Compared with benign group, the enhancement start time and peak time in malignant group were significantly shortened, and the peak intensity significantly increased(all P<0.05). The specificity, accuracy and positive predictive value of CEUS combined with BI-RADS grading for distinguishing malignant and benign lesions were significantly higher than those of two methods alone(all P<0.05). There was no significant difference in sensitivity and negative predictive value among the three diagnostic method groups(both P>0.05). Conclusion The combination of CEUS and BI-RADS grading can increase the diagnostic accuracy for distinguishing benign and malignant breast lesions.
