不同尿沉渣分析仪与显微镜检查的比较和分析

【摘要】 目的比较SYSMEX UF-1000i与SYSMEX UF-5000两种尿沉渣分析仪在白细胞(WBC)、红细胞(RBC)、管型(CAST)检测上的灵敏度、特异性以及与显微镜检查的一致性。方法随机收集2021年2—5月首都医科大学附属北京佑安医院病房及门诊患者1048份尿液标本。分别使用SYSMEX UF-1000i、SYSMEX UF-5000以及显微镜进行检测,超过参考范围为阳性,否则为阴性,记录结果并进行比较分析。结果 SYSMEX UF-1000i检测、SYSMEX UF-5000检测、镜检尿液WBC阳性率分别为20.04%、19.47%、17.56%;RBC阳性率分别为17.37%、14.98%、11.83%;CAST阳性率分别为35.31%、7.44%、6.77%。SYSMEX UF-1000i、SYSMEX UF-5000检测相比较,WBC、RBC结果差异无统计学意义(P> 0.05),CAST结果差异有统计学意义(P <0.05)。与镜检相比较,SYSMEX UF-1000i检测WBC结果差异无统计学意义(P> 0.05),RBC、CAST结果差异有统计学意义(P <0.05);SYSMEX UF-5000检测WBC、CAST结果差异无统计学意义(P> 0.05),RBC结果差异有统计学意义(P <0.05)。SYSMEX UF-5000较SYSMEX UF-1000i检测结果与镜检结果有更好的一致性。结论 SYSMEX UF-5000检测性能优于SYSMEX UF-1000i,尿沉渣分析仪检测结果仍受多种因素影响,需结合显微镜检查提高检测准确性。

【Abstract】 Objective To compare the sensitivity and specificity of SYSMEX UF-1000 i and SYSMEX UF-5000 urine sediment analyzers in the detection of white blood cell(WBC), red blood cell(RBC) and cast(CAST), and their consistency with microscopy. Methods A total of 1048 urine samples were collected randomly from the patients in wards and outpatient of Beijing You’an Hospital Affiliated to Capital Medical University from February to May 2021. SYSMEX UF-1000 i, SYSMEX UF-5000 and microscopic examination were used for detection. If the value exceeds the reference range,it will be positive, otherwise it will be negative, and the results will be compared and analyzed. Results The positive rate of urine WBC in SYSMEX UF-1000 i, SYSMEX UF-5000 and microscopic examination were 20.04%, 19.47%, 17.56%,respectively, the positive rate of RBC was 17.37%, 14.98%, 11.83%, respectively, and the positive rate of CAST was 35.31%, 7.44%, 6.77%, respectively. There were no significant differences in the results of WBC and RBC between SYSMEX UF-1000 i and SYSMEX UF-5000(P > 0.05), but there was significant difference in the results of CAST(P < 0.05). Compared with microscopic examination, there was no significant difference in the results of WBC detected by SYSMEX UF-1000 i(P > 0.05), but there were significant differences in the results of RBC and CAST(P < 0.05), and there were no significant differences in the results of WBC and CAST detected by SYSMEX UF-5000(P > 0.05), and there was significant difference in urine RBC(P < 0.05). Compared with SYSMEX UF-1000 i, the test results of SYSMEX UF-5000 and the results of microscopy had better consistency. Conclusion SYSMEX UF-5000 has better detection performance than SYSMEX UF-1000 i, and the results of urine sediment analyzer are still affected by many factors, so it is necessary to improve the detection accuracy by combining with microscopic examination.
