【摘要】 目的 分析腹壁切口选择对二次分娩剖宫产手术的影响。方法 选取100例第二次剖宫产产妇,根据产妇腹壁切口不同分为观察组与对照组,每组50例。对照组产妇采取横切口方法,观察组产妇采取纵切口方法,评估不同切口对产妇的影响。结果 两组产妇术中出血量、胎儿娩出时间、手术时间比较,差异有统计学意义(P<0.05),两组首次肛门排气时间、泌乳时间比较,差异无统计学意义(P>0.05)。手术前,两组情绪评分、身体舒适度评分、自理能力评分及疼痛评分比较,差异无统计学意义(P>0.05);手术后,两组情绪评分、身体舒适度评分、疼痛评分及腹腔粘连程度比较,差异有统计学意义(P<0.05),两组自理能力评分比较,差异无统计学意义(P>0.05)。结论 纵切口实施剖宫产术的手术时间短,发生腹腔粘连率低,但切口美观度低;横切口的产妇舒适度高,情绪评分及疼痛评分较低,术中出血量较少。临床应根据不同情况合理选择切口实施手术。更多还原

【Abstract】 Objective To analyze the effect of abdominal wall incision selection on caesarean section for second delivery.Methods 100 cases of second caesarean section were selected and divided into observation group and control group according to different abdominal wall incision, with 50 cases in each group. The control group took the method of transverse incision, and the observation group took the method of longitudinal incision to evaluate the influence of different incision on the mothers.Results There were statistically significant differences in the amount of blood loss, fetal delivery time and operation time between the two groups(P<0.05), while there were no statistically significant differences in the first anal exhaust time and lactation time between the two groups(P>0.05). Before operation, there was no significant difference in emotional score, physical comfort score,self-care ability score and pain score between the two groups(P>0.05). After surgery, there were statistically significant differences in emotional score, physical comfort score, pain score and abdominal adhesion degree between the two groups(P<0.05), but no statistically significant differences in self-care ability score between the two groups(P>0.05). Conclusion The operation time of cesarean section with longitudinal incision is short, the adhesion rate of abdominal cavity is low, but the incision aesthetics is low.The women with transverse incision had higher comfort, lower emotional score and pain score, and less intraoperative blood loss.Clinically, incision should be selected reasonably according to different conditions.更多还原
