痛泻安肠方调控肠易激综合征小鼠Th17/Treg细胞免疫平衡的研究

【摘要】 目的 观察痛泻安肠方治疗腹泻型肠易激综合征(IBS-D)(肝郁脾虚证)小鼠及对辅助性T细胞17(Th17)/调节性T细胞(Treg)细胞免疫平衡的影响。方法 将36只C57BL/6小鼠随机分为空白组、模型组、匹维溴铵组和痛泻安肠方低、中、高剂量组,每组6只,采用束缚应激联合番泻叶灌胃建立IBS-D(肝郁脾虚证)小鼠模型,匹维溴铵组予匹维溴铵混悬液灌胃,痛泻安肠方低、中、高剂量组分别予4.78、9.56、19.12 g/kg痛泻安肠方溶液灌胃,模型组予0.9%氯化钠溶液灌胃,连续14 d。观察小鼠的一般状况、体质量变化及粪便性状,计算小鼠粪便含水量;采用腹壁撤退反射评分评估小鼠内脏敏感性;采用HE染色观察小鼠结肠组织病理变化;流式细胞术检测肠系膜淋巴结Th17和Treg比例。结果 与空白组比较,模型组小鼠出现不同程度的情绪低落、大便稀溏等症状,体质量显著偏低,Bristol粪便性状量表评分及粪便含水量、腹壁撤退反射评分均显著升高,组织病理学观察未见明显差异,肠系膜淋巴结Th17占比显著升高,Treg占比显著下降(P <0.05或P <0.01);经不同剂量痛泻安肠方治疗后,粪便含水量显著下降,中、高剂量组Bristol粪便性状量表评分、腹壁撤退反射评分均显著降低,各剂量组肠系膜淋巴结Th17占比显著降低,Treg占比显著回升(P <0.05或P <0.01)。结论 痛泻安肠方对IBS-D(肝郁脾虚证)小鼠具有较好的治疗效果,其作用机制可能与恢复Th17/Treg细胞平衡有关。

【Abstract】 Objective:To observe the effect of Tongxie Anchang Decoction on mice with hepatic depression and spleen deficiency diarrhea type irritable bowel syndrome(IBS-D)and its effect on the immune balance of helper T cell 17(Th17)/regulatory T cells(Treg). Methods:36 C57BL/6 mice were randomly divided into blank group,model group,pinaverium bromide group,Tongxie Anchang Decoction group,6 mice in each group. Establishment of liver depression and spleen deficiency type IBS-D mouse model was by restraint stress combined with senna leaf gavage.The pinaverium bromide group was administered with pinaverium bromide suspension by gavage. The lowdose,middle-dose and high-dose groups were given 4.78,9.56,19.12 g/kg Tongxie Anchang Decoction by gavage,respectively. The model group was gavaged with 0.9% sodium chloride solution for 14 d. The general condition,body mass and fecal properties of the mice were observed,and the fecal water content of the mice was calculated;the visceral sensitivity of the mice was assessed by the abdominal wall withdrawal reflex score;the histopathological changes of the colon of the mice were observed by HE staining. Flow cytometry was used to detect the ratio of Th17 and Treg in peripheral blood. Results:Compared with the blank group,the mice in the model group had different degrees of depression,loose stool texture and other symptoms. The were significantly lower in body weights(P < 0.05 或 P < 0.01). The Bristol Fecal Trait Scale scores,fecal water content,and abdominal wall withdrawal reflex scores were significantly increased,and there was no significant difference in histopathological observation.The proportion of Th17 in mesenteric lymph nodes was significantly increased,and the proportion of Treg was significantly decreased;After different doses of Tongxie Anchang Decoction treatment,the water content of feces was significantly decreased,and the scores of Bristol Fecal Trait Scale and abdominal wall withdrawal reflex scores were significantly decreased in the middle and high dose groups. In each dose group,the proportion of Th17 in mesenteric lymph nodes decreased significantly,and the proportion of Treg increased significantly(P < 0.05 或 P <0.01). Conclusion:Tongxie Anchang Decoction has better therapeutic effect on liver-depressed and spleen-deficient IBS-D mice,and its mechanism of action may be related to the restoration of Th17/Treg cell balance.
