【摘要】 目的：分析江苏省城乡基层医生老年健康管理能力现状及其影响因素，为提升基层医疗机构老年健康管理服务能力提供决策参考依据。方法：于2020年8月，利用多阶段分层抽样和典型抽样结合的方法，对江苏省2912名基层医生进行问卷调查。调查内容涵盖基层医生的基本素质、老年健康管理能力具备情况等方面。通过二元Logistic回归模型探究基层医生老年健康管理能力的影响因素。结果：有2545名城乡基层医生具备5种及以上的老年人健康管理能力，总具备率为87.40%;城乡基层医生掌握心脑血管疾病评分标准的医生比例最低，仅占54.05%和57.94%;农村医生各项老年人健康管理能力具备率均高于城市地区(90.73%vs 83.64%,P<0.05);最高学历、工作机构类型和执业注册类别是影响江苏省基层医生老年健康管理服务能力的重要因素(P<0.05)。结论：江苏省基层医生队伍中高素质人才不足，农村地区基层医生年龄结构老化且教育水平偏低；基层医生老年健康管理服务能力总体较好，心脑血管意外应急处理能力仍需加强；在政策引导下需拓展基层医生职业发展前景和待遇、落实全科医学专业培养以及进一步加强基层医生专业知识与技能的规范化培训，并结合基层医生个人服务能力影响特征，因地制宜地给与更多支持。更多还原

【Abstract】 Objective To analyze the status and influencing factors of geriatric health management ability of primary care physicians in urban and rural areas in Jiangsu Province, and provide a decision-making basis for improving the ability of geriatric health management services in primary care institutions. Methods In August 2020, a combined sampling method of multi-stage stratified sampling and typical sampling was used to conduct questionnaire survey among primary care physicians in Jiangsu Province. The survey content included the basic characteristics of primary care doctors and the availability of geriatric health management competence. A binary logistic regression model was used to explore the factors associated with the geriatric health management ability of primary care physicians. Results There were 2,545 urban and rural primary care physicians having five or more geriatric health management abilities, with an overall rate of 87.40%. The proportion of urban and rural primary care physicians equipped with scoring criteria for cardiovascular disease was the lowest, only accounting for 54.05% and 57.94%, respectively. The availability rate of every geriatric health management competency of rural physicians was higher than that of urban areas(P<0.05). The highest educational level, type of work institution, and category of license registration were significant factors associated with the ability of primary care physicians’ geriatric health management services in Jiangsu Province(P<0.05). Conclusion High-quality talents were insufficient in the primary care physician team in Jiangsu Province, and primary care physicians in rural areas had an aged population composition and a low level of education. Overall, the ability of geriatric health management services delivered by primary care doctors was relatively good, while the ability to deal with cardiovascular and cerebrovascular emergencies needed to be improved. Guided by the policy, it is necessary to broaden the career development prospects and remuneration of primary care doctors, implement specialized training of general medicine, strengthen the standardized training of primary care doctors in professional knowledge and skills, and provide more support to primary care doctors according to the factors influencing their individual service capabilities.更多还原
