【摘要】 目的　探讨尿乳酸 /肌酐比值与新生儿缺氧缺血性脑病 (HIE)的关系。方法　选择有窒息史的新生儿 4 0例为观察组 ,按病情分为轻度HIE组、中重度HIE组 ;正常足月新生儿 38例为对照组。用生化法测定生后 6h内及 4 8～ 72h内尿乳酸 /肌酐比值。结果　生后 6h内尿乳酸 /肌酐比值中重度HIE组显著高于正常组和轻度HIE组 (P <0 .0 1)。以 0 .4 5为界值判断中重度HIE发生敏感度为 93.3% ,特异度为 98.3%。结论　生后6h内尿乳酸 /肌酐比值对判断中重度HIE的发生有高度的敏感性和特异性 ,可作为一种快速、经济的早期诊断方法更多还原

【Abstract】 Objective To study the relationship between the urinary lactate to creatinine ratio and neonate with hypoxic-ischemic encephalopathy(HIE). Methods Fourty infants with asphyxia were selected as study group and were divided into the mild encephalopathy group, the moderate and the severe encephalopathy group,according to the severity of the diseases.Thirty-eight normal term infants were selected randomly as control group.The ratio of lactate to creatinine in urine was measured with biochemical method within 6 hours,and 48～72 hours repeatedly after birth. Results Within 6 hours after birth, the ratio of urinary lactate to creatinine in the moderate and the severe encephalopathy group was distinctly higher than that in the mild encephalopathy group and that in normal infants(P<0.01).With 0.45 being the boundary value , the ratio of 93.3 % sensitivity and of 98.3 % specificity was determined as the moderate or the severe HIE respectively.Conclusions Measurement of the urinary lactate to creatinine ratio within 6 hours after birth has high sensitivity and specificity in predicting the development of the moderate or the severe HIE, which is a rapid and economic early stage diagnostic method.更多还原
