血清CA199、CEA与非酒精性脂肪肝的相关性

【摘要】 目的 分析非酒精性脂肪肝(NAFLD)患者血清中癌抗原199(CA199)、癌胚抗原(CEA水平,明确其对NAFLDA的诊断及对肝功能的影响。方法 选取2022年1月至2023年12月首都医科大学附属北京天坛医院收治的104例NAFLD患者为NAFLD组,选取同期80名正常健康体检者作为对照组。采用全自动化学发光免疫分析法检测两组CA199、CEA水平;采用Pearson法分析CA199、CEA的相关性以及二者与ALT、γ-GT、AST的相关性;绘制ROC曲线分析CA199、CEA对NAFLD的诊断价值;采用Logistic回归分析影响NAFLD发生的多因素。结果 NAFLD组TG、TC、LDL-C、血糖水平均显著高于对照组,HDL-C水平显著低于对照组,差异有统计学意义(P<0.05);NAFLD组血清CA199、CEA水平显著高于对照组,差异有统计学意义(P<0.05),CA199、CEA二者水平具有正相关关系(P<0.05);ROC曲线显示,CA199、CEA二者联合诊断的AUC高于两指标单独诊断的AUC值(P<0.05);NAFLD组ALT、γ-GT、AST水平高于对照组,差异有统计学意义(P<0.05);血清CA199、CEA水平与ALT、γ-GT、AST均呈正相关(P<0.05)。TC(OR=2.152)、CA199(OR=2.644)、CEA(OR=4.154)水平均是NAFLD发生的影响因素(P<0.05)。结论 NAFLD患者血清中CA199、CEA水平显著升高,二指标与肝功能密切相关,对NAFLD诊断具有一定的临床价值。

【Abstract】 Objective To analyze the serum levels of cancer antigen 199(CA199) and carcinoembryonic antigen(CEA) in patients with nonalcoholic fatty liver disease(NAFLD) and to clarify their effects on the diagnosis of NAFLD and liver function. Methods A total of 104 patients with NAFLD were admitted to Beijing Tiantan Hospital Affiliated with Capital Medical University from January 2022 to December 2023 and selected as the NAFLD group. Additionally, 80 normal healthy subjects during the same period were chosen as the control group. The levels of CA199 and CEA were detected using automatic chemiluminescence immunoassay. The correlations between CA199, CEA, ALT, γGT and AST were analyzed using the Pearson method. An ROC curve was created to assess the diagnostic value of CA199 and CEA in NAFLD. Multiple factors influencing the occurrence of NAFLD were analyzed using logistic regression. Results TG, TC, LDL-C, and blood glucose levels in the NAFLD group were significantly higher than those in the control group, while HDL-C levels were significantly lower than those in the control group, with statistical significance(P<0.05). Serum CA199 and CEA levels in the NAFLD group were significantly higher than those in the control group, the difference was statistically significant(P<0.05). There was also a a positive correlation between CA199 and CEA levels(P<0.05). The ROC curve showed that the AUC value of combined CA199 and CEA diagnosis was higher than that of CA199 and CEA alone(P<0.05).ALT,γ-GT,and AST levels in the NAFLD group were significantly increased compared to the control group,with statistical significance(P<0.05).Serum CA199 and CEA levels were positively correlated with ALT,γ-GT,and AST levels(P<0.05).It was determined that TC(OR=2.152),CA199 (OR=2.644) and CEA (OR=4.154) levels were all influential factors in the occurrence of NAFLD (P<0.05).Conclusion Serum levels of CA199 and CEA were significantly higher in patients with NAFLD.These two indicators were closely related to liver function,and had clinical value in diagnosing NAFLD.
