CT与磁共振成像对脊柱结核临床诊断价值分析
Diagnostic value of CT and magnetic resonance imaging in spinal tuberculosis
【摘要】 目的 探讨CT与磁共振成像对脊柱结核临床诊断价值。方法 入选2018年1月~2021年12月为我院收治的疑似脊柱结核患者72例。分别对其实施CT检查(CT组72例)和磁共振成像检查(MRI组72例)。将临床病理检查结果 作为金标准,对比两种检查方式的诊断准确率。结果 本组病理检查结果 :椎体、椎间盘破坏72例(100%)、死骨钙化63例(87.50%)、韧带下扩散68例(94.44%)、脊膜强化52例(72.22%)、硬膜外或椎间孔侵犯22例(30.56%);死骨钙化方面CT组诊断率80.95%(51/63),MRI组7.94%(5/63),差异有显著性性(P<0.05);韧带下扩散、脊膜强化、硬膜外或椎间孔侵犯诊断准确率:MRI组均高于CT组,组间差异具有统计学意义(P<0.05)。结论 在对脊柱结核患者实施临床诊断的过程中,选用CT诊断技术及磁共振成像诊断技术均有一定优势和缺陷,需要相关人员能够结合患者的症状合理选取诊断措施,如有必要可二者联合使用。
【Abstract】 Objective To explore the clinical diagnostic value of CT and magnetic resonance imaging in spinal tuberculosis.Methods 72patients with suspected spinal tuberculosis admitted to our hospital from January 2018 to December 2021 were enrolled.CT examination(72 cases in CT group) and magnetic resonance imaging(72 cases in MRI group) were performed respectively.The clinicopathological examination results were used as the gold standard to compare the diagnostic accuracy of the two methods.Results In this group,the pathological examination results were as follows:72 cases of vertebral body and intervertebral disc destruction(100%),63 cases of dead bone calcification(87.50%),68 cases of subligamental diffusion(94.44%),52 cases of meningeal enhancement(72.22%),22 cases of epidural or foraminal invasion(30.56%);The diagnostic rate of dead bone calcification was 80.95%(51/63) in CT group and 7.94%(5/63) in MRI group,and the difference was significant(P<0.05).The diagnostic accuracy of subligament diffusion,meningeal enhancement,epidural or foraminal invasion was higher in MRI group than in CT group,and the difference between groups was statistically significant(P<0.05).Conclusion In the process of clinical diagnosis of patients with spinal tuberculosis,CT diagnostic technology and magnetic resonance imaging diagnostic technology have certain advantages and disadvantages,and relevant personnel should be able to reasonably select diagnostic measures based on the symptoms of patients,if necessary,they can be used in combination.
