辛润宣通法治疗神经病理性疼痛细胞因子的调控机制

【摘要】 目的 分析辛润宣通法治疗神经病理性疼痛细胞因子的调控机制。方法 选取SPF级Wistar大鼠120只,分为假手术组、模型组、旋覆花汤高剂量组、中剂量组及低剂量组、普瑞巴林组6组,每组20只,假手术组正常喂养,剩余5组建立神经病理性疼痛大鼠模型。建模成功后旋覆花汤高剂量组、中剂量组、低剂量组分别给予10 g·kg-1、5 g·kg-1、2.5 g·kg-1旋覆花汤药液,普瑞巴林组给予15 mg·kg-1普瑞巴林药液,假手术组及模型组同量0.9%氯化钠溶液,每日1次,15 d后观察大鼠变化。结果 与假手术组相比,模型组、普瑞巴林组、旋覆花汤低剂量组、中剂量组及高剂量组自发性疼痛评分、IL-1β、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)、5-羟色胺(5-hydroxytryptamine,5-HT)、PGE2水平、TLR4、NF-κB mRNA表达量及相对表达量上升,MWT、TWL、IL-10、β-EP水平下降(P<0.05);与模型组相比,普瑞巴林组、旋覆花汤低剂量组、中剂量组及高剂量组自发性疼痛评分、IL-1β、TNF-α、5-HT、PGE2水平、TLR4、NF-κB mRNA表达量及相对表达量下降,MWT、TWL、IL-10、β-EP水平上升(P<0.05);与普瑞巴林组、旋覆花汤低剂量组、中剂量组相比,旋覆花汤高剂量组自发性疼痛评分、IL-1β、TNF-α、5-HT、PGE2水平、TLR4、NF-κB mRNA表达量及相对表达量下降,MWT、TWL、IL-10、β-EP水平上升(P<0.05)。结论 神经病理性疼痛大鼠经辛润宣通法治疗后炎性因子水平得到调节,疼痛减轻,机械刺激及热刺激敏感性改善,且呈剂量依赖,其机制可能与TLR4/NF-κB信号通路得到抑制有关。

【Abstract】 Objective To analyze the regulatory mechanism of cytokines involvement in neuropathic pain treated by Xinrun Xuantong method.Methods A total of 120 SPF Wistar rats were selected and divided into six groups,namely the sham operation group,the model group,Xuanfuhua decoction high-dose group,medium-dose group,low-dose group,and pregabalin group,with20 rats in each group.The sham operation group was fed normally,and the remaining five groups were established neuropathic pain rat models.After successful modeling,Xuanfuhua decoction high-dose group,medium-dose group and low-dose group were given 10 g·kg-1, 5 g·kg-1,and 2.5 g·kg-1 Xuanfuhua decoction respectively,pregabalin group was given 15 mg·kg-1 pregabalin solution,the sham operation group and the model group were given the same amount of 0.9% sodium chloride solution,once a day,and after 15 days,the changes of rats were observed.Results Compared with the sham operation group,spontaneous pain score,the levels of interleukin(IL)-1β,tumor necrosis factor-α(TNF-α),5-hydroxytryptamine(5-HT) and prostaglandin E2(PGE2),and expression levels of Toll like Receptor 4(TLR4) and nuclear factor-κB(NF-κB) mRNA increased in the model group,pregabalin group,Xuanfuhua decoction low-dose group,medium-dose group and high-dose group,and the levels of mechanical withdrawal threshold(MWT),thermal withdrawal latency(TWL),IL-10 and β-endorphin(β-EP) were decreased(P<0.05);Compared with the model group,spontaneous pain scores,levels of IL-1β,TNF-α,5-HT and PGE2,and expression levels of TLR4 and NF-κB mRNA in pregabalin group,Xuanfuhua decoction low-dose group,medium-dose group and high-dose group were decreased,and the levels of MWT,TWL,IL-10 and β-EP were increased(P<0.05);Compared with pregabalin group,Xuanfuhua decoction lowdose group and medium-dose group,the spontaneous pain score,levels of IL-1β,TNF-α,5-HT and PGE2,and expression levels of TLR4 and NF-κB mRNA in Xuanfuhua decoction high-dose group were decreased,and the levels of MWT,TWL,IL-10 andβ-EP were increased(P<0.05).Conclusion The levels of inflammatory factors in neuropathic pain rats are regulated after treatment with Xinrun Xuantong method,pain is reduced,and the sensitivity to mechanical and thermal stimulation is improved in a dosedependent manner.The mechanism may be related to the inhibition of TLR4/NF-κB signaling pathway.
