声脉冲辐射力成像对精索静脉曲张不育患者睾丸的弹性定量研究

【摘要】 目的运用声脉冲辐射力成像技术(ARFI)探讨左侧精索静脉曲张(VC)引起男性不育患者睾丸组织硬度改变与精液质量之间的关系。方法将我院就诊的89例左侧精索静脉曲张患者(VC组)分为VC1级(30例),VC2级(42例),VC3级(17例)3个亚组,另设正常对照组40例。对VC组患者使用常规超声检查精索静脉曲张程度,对所有受检者使用声脉冲辐射力成像技术测定睾丸剪切波速度(Vs)。精液做常规检查。结果与对照组比较,VC组及VC1~VC3组左侧睾丸组织Vs值降低(P<0.05),而精子密度与精子活率也降低(P<0.05、P<0.01);VC组及VC2~VC3组右侧睾丸组织Vs值降低(P<0.05)。相关分析显示,VC组睾丸组织Vs值与精子密度、活率有一定的正相关(r=0.453,P=0.025;r=0.617,P=0.041)。结论运用ARFI技术测量VC患者睾丸组织Vs值,可一定程度内反映睾丸质地变化及睾丸的生精功能。

【Abstract】 Objective To investigate the relationship between the change of testicular tissue stiffness and the semen quality of male infertile patients caused by left varicocele(VC)by using acoustic radiation force impulse imaging(ARFI).Methods 89 cases of patients with left varicocele(89 VC group)in our hospital were divided into grade VC1(30 cases),grade VC2(42 cases),VC3 grade(17 cases),and in addition,40 cases of normal control group were added.The degree of varicocele in the VC group were examined by conventional ultrasound.The shear wave velocity in testes of all subjects were determined by acoustic radiation force impulse.The routine examinations of semen were performed.Results Compare with the control group,Vs value of left testis tissue decreased in the VC group and VC1-VC3 group(P<0.05),and sperm density and sperm motility were also decreased(P<0.05,P<0.01),and Vs value of right testis tissue decreased in the VC group and VC2-VC3 group(P<0.05).The linear correlation analysis showed that there was a positive correlation between Vs value and sperm density and motility in VC group(r=0.453,P=0.025;r=0.617,P=0.041).Conclusions Vs value of the testis tissue in patients with varicocele can reflect the change of testicular texture and spermatogenesis function in a certain degree by using ARFI technology.
