基于颈椎成熟分期法的东北地区人群下颌骨生长发育特点研究
The growth and development pattern of mandible based on cervical vertebral maturation stage in Northeast China
【摘要】 目的 研究东北地区人群下颌骨随颈椎成熟分期的生长发育模式,分析下颌骨三维方向生长发育的协调性,并探索下颌骨生长发育的性别间差异。方法 随机选取2017~2022年在吉林大学口腔医院正畸科就诊的420例籍贯为东北地区的患者,依据性别与颈椎成熟分期(cervical vertebral maturation stage,CVMS)进行分组,应用Dolphin 11.9头影测量软件标定头颅定位正侧位片中11个下颌骨相关标志点并测量计算14个线性、角度及比例项目,使用SPSS 26.0进行统计分析。结果 (1)多数测量项目与颈椎成熟分期之间呈现显著的相关性;(2)下颌骨随着颈椎不断成熟在三维方向上均有显著生长,多数测量项目的增长集中在CVMS II~CVMS IV阶段(P<0.05),且女性生长发育高峰通常比男性更早结束;(3)下颌骨三维方向的生长发育呈现非均衡生长模式,其中男性下颌骨早期三维生长速度大致为:垂直向>水平向=矢状向,而女性发育早期为:垂直向=矢状向>水平向,而后三向基本协调生长;(4)在颈椎分期同一阶段,男女性在三维方向上的增幅及增速存在显著的性别差异(P<0.05)。结论 东北地区人群下颌骨的生长发育呈现与颈椎成熟分期相关的发育特点且不同性别呈现相似但有差异的发育模式。
【Abstract】 Objective To investigate the growth and development pattern of mandible with cervical vertebral maturation stage(CVMS) in Northeast China, associated with analyzing the coordination of mandible development in threedimensions,and explore the gender differences on the mandible development.Methods A total of 420 patients who came from Northeast China were chosen randomly from the visitors to the Department of Orthodontics Hospital of Stomatology Jilin University from 2017 to 2022. The enrolled participants were grouped by gender and CVMS. A total of 11 landmarks and 14 items were pointed and measured via Dolphin software(version 11.9). One-way ANOVA and T-test were employed for statistical analysis using SPSS(version 26.0).Results (1) There was a significant correlation between most of the measurement items and CVMS;(2) Most of the growth measurements were concentrated in the CVMS Ⅱ to CVMS Ⅳ stage, and the growth peak of female usually ends earlier than that of male;(3)The mandible development showed an unbalanced growth pattern. In the early stage, the growth rate was about vertical > horizontal = sagittal in male, while the growth rate was vertical = sagittal > horizontal in female;(4)At the same CVMS, there were significant gender differences in the increase rate of mandible growth(P<0.05).Conclusion The growth and development of mandible in Northeast Chinese population showed developmental characteristics related to CVMS, and similar but different developmental patterns were observed between males and females.
