倍他司汀治疗眩晕症的临床疗效及对脑血流动力学的影响研究

【摘要】 目的:从药理学机制及安全性价值两方面探讨倍他司汀的临床应用效益,及在眩晕症干预中的价值。方法:随机分组路径下眩晕症患者共60例,分为两组各30例,即JC组和JC+B组。JC组开展基础致病因素对症治疗+盐酸氟桂利嗪口服+天麻素注射液静脉滴注治疗,JC+B组在JC组的基础上联合应用倍他司汀。比较JC组和JC+B组患者干预前后临床症状积分的改善效果、临床疗效、血液流变学指标(CP、PV、FIB)、脑血流动力学指标(左、右椎动脉和基底动脉血流速度)及药物不良反应。结果:治疗7d、14d,JC+B组患者眩晕症状总积分低于JC组(P <0.05);JC+B组临床总有效率为90%,高于JC组的76.67%(P <0.05)。治疗后,JC+B组CP、PV、FIB均低于JC组(P <0.05),左、右椎动脉和基底动脉血流速度均高于JC组(P <0.05)。在用药安全性监测上,JC+B组共13.33%的不良反应率,与JC组的10%比较,P> 0.05。结论:倍他司汀的主要作用机理在于改善血液流变学,于眩晕症患者应用可有效改善患者脑血流动力学,从而增加脑组织血流灌注及供血,有效解除眩晕症基础病理因素,提高临床疗效,且药物安全性良好。

【Abstract】 Objective: To explore the clinical application benefit of betahistine and its value in the intervention of vertigo from the aspects of pharmacological mechanism and safety value. Methods: Vertigo patients with 60 cases were randomly divided into two groups, JC group and JC+B group, with 30 cases in each group. JC group was treated with symptomatic treatment of basic pathogenic factors, oral flunarizine hydrochloride and intravenous infusion of gastrodin injection, while JC+B group was combined with betahistine on the basis of JC group. The changes of vertigo symptom total score, clinical efficacy, hemorheological indexes(CP, PV, FIB), cerebral hemodynamic indexes cerebral tissue arterial blood flow velocity) and pharmacological safety benefits of drugs were compared. Results: After 7 days and 14 days of treatment, JC+B group was better than JC group in the improvement of the total score of vertigo symptoms(P<0.05). The clinical efficacy rate of JC+B group was 90% vs 76.67%, and the efficacy of JC+B group was more advantageous(P<0.05). After treatment, the improvement of CP, PV and FIB in JC+B group was more advantageous(P<0.05). In the improvement effect of cerebral artery blood flow velocity, JC+B group was more you’shi than JC group(P<0.05).In the pharmacologic safety monitoring(adverse reaction rate), the VSJC group in JC+B group was 13.33%vs 10%, compared with P>0.05. Conclusion: The application of betahistine in the treatment of vertigo can improve the cerebral hemodynamics on the basis of improving the hemorheology of patients, so as to increase the cerebral blood perfusion and blood supply, effectively relieve the basic pathological factors of vertigo, improve the clinical efficacy, and the drug safety is good.
