【摘要】 目的探讨完全经乳晕入路甲状腺手术的临床疗效。方法选择本院2012年1~12月收治的甲状腺肿瘤患者91例,三孔入路腔镜下采用超声刀游离胸前区以及颈前肌群,用超声刀将腺体或腺叶切除,并进行血管处理。结果 91例甲状腺切除手术成功完成,其中,2例患者冰冻切片提示甲状腺癌,中转行中央区淋巴清扫,术后给予131I治疗。手术时间为30~120 min,平均75 min。术中出血量为10~90 ml,术后6 h能下床活动,主要食用流质食物,颈部引流管在术后第2天拔除,术后4 d左右出院。其中2例患者出现皮下淤斑,3 d后消失。结论完全经乳晕入路腔镜下甲状腺手术具有良好的疗效,手术安全性高,切口隐蔽,没有瘢痕,值得推广。更多还原

【Abstract】 Objective To investigate the efficacy of thyroid operation completely through breast areola approach. Methods 91 cases of thyroid cancer patients from January to December in 2012 were selected, from triporate approach, ultrasonic knife was used to dissociate chest and anterior cervical muscle, the thyroid gland or glands leaves were resected with ultrasonic scalpel, and given vascular treatment. Results 91 cases of thyroid resection operation completed successfully, 2 patients were found with thyroid cancer by frozen section and tuned to lymph node dissection, and given 131I therapy after surgery. Operation time was 30-120 minutes, average 75 minutes. Bleeding in surgery was 10-90 ml, six hours after operation, the patients were able on activities, and major eating liquid food, drainage tube in the neck was pulled out on the next day after the operation, patients discharged on 4 days after surgery. There were 2 cases of subcutaneous ecchymosis, disappeared 3 days later. Conclusion The efficacy of thyroid operation completely through breast areola approach is significant, with high safety, no scar, and worthy of clinical application.更多还原
