【摘要】 目的 探讨内囊预警综合征的临床特点及动脉局部应用替罗非班的治疗效果。方法 选取2020年1～12月于海南医学院第一附属医院住院治疗的内囊预警综合征患者10例，总结患者的临床特点和治疗效果。结果 本研究共纳入10例患者，其中2例在术前表现为短暂性脑缺血发作，8例表现为进展性卒中。所有患者均存在缺血性脑血管病发病的危险因素，其中5例无颅内血管狭窄。所有患者均采用动脉局部注射替罗非班氯化钠注射液治疗，术后患者的美国国立卫生研究院卒中量表评分不同程度降低，所有患者均未出现不良反应。根据患者治疗第90天的改良Rankin量表（modified Rankin scale,mRS）评分，7例患者预后良好（mRS评分0～2分），2例患者预后不良（mRS评分≥3分），1例患者失访。结论 颅内小血管病变可能是内囊预警综合征发病的主要机制，动脉局部应用替罗非班不增加不良反应，可在一定程度上促进患者神经功能恢复。更多还原

【Abstract】 Objective To explore the clinical characteristics of capsular warning syndrome(CWS) and the effectiveness of local arterial application of tirofiban. Methods Ten patients with CWS hospitalized in the First Affiliated Hospital of Hainan Medical College from January to December 2020 were selected to summarize the clinical characteristics and therapeutic effects of the patients. Results A total of 10 patients were included in this study, two of them presented with transient ischemic attack before operation, and eight presented with progressive stroke. There were risk factors of cerebrovascular diseases in the ten patients with CWS, while in which five patients without intracranial vascular stenosis. All patients were treated with local arterial injection of tirofiban sodium chloride injection. The National Institutes of Health stroke scale scores of all patients were reduced to varying degrees after surgery, and no adverse reactions were reported in all patients. According to the modified Rankin scale(mRS) score on day 90 of treatment, seven patients had good prognosis(mRS score 0-2), two patients had poor prognosis(mRS score ≥3),and one patient was lost to follow-up. Conclusion Intracranial small vessel lesions may be the main mechanism of CWS. The local arterial application of tirofiban does not increase adverse reactions, and can promote the recovery of neurological function in patients to a certain extent.更多还原
