【摘要】 目的：研究电针对全膝关节置换术后功能康复的影响。方法：采用严格的随机对照临床研究方法，将120例全膝关节置换术后的患者随机分为电针治疗组和常规治疗组。对比两组患者基线资料，采用美国特种外科医院膝关节评分标准(hospital for special surgery knee score, HSS评分)、视觉模拟评分(Visual Analogue Score, VAS)、PCNA药物用量进行疼痛和功能评估，测量膝关节轴径、观察术后恶心呕吐、并发症等指标。结果：(1)电针治疗组与常规治疗组VAS评分，在术后第一天，两者之间差异无统计学意义(P>0.05);但在术后三天、五天差异有统计学意义(P<0.05)。术后两天内PCNA药物使用量电针治疗组低于常规治疗组，差异有统计学意义(P<0.05)。(2)术后第三天、第五天，HHS评分电针治疗组明显高于常规治疗组，差异有统计学意义(P<0.01)。(3)两组在并发症比较中，电针治疗组无晕针、断针、滞针发生，电针治疗组恶心呕吐发生率低于常规治疗组，差异有统计学意义。结论：电针在全膝关节置换术后镇痛、改善肢体肿胀方面表现优异，由于其可间接减少镇痛药物，也降低了患者术后恶心呕吐的并发症，明显促进患者功能康复，作用安全可靠。更多还原

【Abstract】 Objective To study the effect of electroacupuncture on functional rehabilitation after total knee arthroplasty.Methods: 120patients after total knee arthroplasty were randomly divided into electroacupuncture group and routine group by strict randomized controlled clinical study.Baseline data of the two groups were compared. The Hospital for Special Surgery knee score(HSS score), Visual Analogue score(VAS) and dosage of PCNA were used to evaluate the pain and function. The axial diameter of knee joint was measured, and postoperative nausea and vomiting and complications were observed. Result(1) There was no significant difference in VAS score between the EA group and the routine group on the first postoperative day(P> 0.05). However, the difference was statistically significant at 3days and 5days after operation(P<0.05). The dosage of PCNA in the EA group was lower than that in the routine group within two days after operation, and the difference was statistically significant(P<0.05).(2) On the third and fifth day after operation, the HHS score of the EA group was significantly higher than that of the routine group, and the difference was statistically significant(P<0.01).In the comparison of complications between the two groups, no needlesickness, broken needle and delayed needle occurred in the EA group. The incidence of nausea and vomiting in the EA group was lower than that in the group, and the difference was statistically significant.Conclusion Electroacupuncture has excellent performance in analgesia and improving limb swelling after total knee arthroplasty.Because it can indirectly reduce the analgesic drugs, also reduce the postoperative nausea and vomiting complications, significantly promote the functional rehabilitation of patients, the effect is safe and reliable.更多还原
